Overview of Staff Professionals and Training

Facility professionals consist of Administrations, RNs, LPNs, Medical Directors, NPs, a Social worker, and two outside entities: 360 Care for vision, dental, podiatry, and audiology services, and Guidestar for psychiatric services. We have multiple in-services through the week and are required to complete online training/education called Relias monthly.

Measures for Ensuring Safety and Well-being

Residents are monitored closely by all staff. Any resident who is at risk for wandering has a wander guard placed on their ankle to sound an alarm if they are close to an exit.

Resources and Support Services for Transition

Residents are encouraged to participate in activities to get to know other residents with similar interests. Staff and the social worker are available to talk to at any time.

Transportation Services

Yes, we have a facility van with a designated transport employee.

Dining Experience

Residents are encouraged to eat in the dining room with other residents. When first admitted, residents will be given the opportunity to go over their likes and dislikes to ensure that they get foods that they like on their trays. CNA's serve out the trays in the dining room and will assist residents with their meals as needed. If the resident were to choose to eat in their room, they would also have that option. If they do not like what is served that day, they can ask for the alternative, which is usually soup, salad, and a sandwich.

Process for Addressing Complaints or Issues

A grievance form will be filed with the administrator, and the interdisciplinary team will discuss solutions to the issue in the morning meeting. We strive to do the best we can at keeping our residents and their families happy.

Cost of Living

$205 a day but we do accept most insurances.

Communication with Residents' Families

Families are welcome to come in or call for information on their loved ones at any time. If there is a change, a concern, or they are sent out, the family will be contacted as soon as possible.

Short-term or Respite Stays

Yes, at the same cost of $205 per day.

Pet-friendly Policy

Yes, the pet may be approved for visits by the administrator. The pet must be up to date on vaccines.

Waitlist Status

We do not have a waitlist, but that is subject to change.

Covid and Flu Vaccinations

Yes, we do provide vaccines.

Yes.
