The Polonaise Assisted Living
The Polonaise assisted living provides an individualized care plan developed through a comprehensive assessment involving you and your care providers (physicians and family). The service-level system concentrates on your personal care support with a range of services that offer flexibility and meet your specific needs and desires.
Floor Plans
Choose from a variety of one and two bedroom floor plans with a full kitchen and private bathroom with walk-in shower. Select apartments feature a balcony or patio with scenic views of Wilson Park and/or walk-in closets. The monthly rate includes utilities, meals, and up to 3 hours of service weekly.
Monthly Rental Rates

- Two Bedroom Apartment 825 sq. ft. $4,385
Monthly Care Level Rates
- Level 1 (up to 3 hours of care per week) Included in monthly rate
- Level 2 (4 - 7 hours of care per week) $700/month
- Level 3 (8 - 14 hours of care per week) $1,525/month
- Level 4 (15 - 21 hours of care per week) $2,350/month
- Level 5 (22 - 28 hours of care per week) $3,175/month

Overview of Types of Professionals on Our Staff and Their Training
- Dietary: Under the direction of the home office head of dietary.
- LPNs: Licensed and under the direction of the home office head of clinical.
- ED and Assistant ED: Trained by the home office regional director and CBRF certified.
- Caregivers: Some are CNAs while others are not, all are trained by our staff.
Measures to Ensure Safety and Well-Being of Residents
- The building is secured and requires a fob or key to enter.
- All residents wear a pendant which they can push when they need assistance or have an emergency.
Resources and Support Services for Resident Transition
- We have a Spiritual Wellness Director from the home office available to help with transitions.
- All staff are sensitive to residents needing time to transition and will address their questions and concerns.
Dining Experience
Our dining experience is restaurant style, allowing residents to take any open seat. A dietary aid will take their drink order followed by their meal order. Alternates are available if residents do not want the menu item for that day. We do not provide specialized diets.
Process for Addressing Resident Complaints or Issues
Residents need to bring their complaints or issues to the attention of the director or assistant director. The issue will be investigated and the resident will be informed about the resolution.
Communication with Residents' Families
Communication is done by phone or by email. Residents need to list on their admission documents who we are allowed to speak with. Generally, a single “point person” is notified about falls, emergencies, etc.
Short-term or Respite Stays
We offer short-term or respite stays. The cost is between $200-220/day depending on the level of care.
Pet-Friendly Policy
We are a pet-friendly community. Pets must be 25 lbs or less, with a limit of two pets. Residents are responsible for the proper care of their pets. Extra costs for pets are included in the packet previously sent.
Vaccinations
Residents can receive Covid and flu vaccinations within the community. We host a clinic every year for these shots.
Aging in Place
We are an RCAC assisted living facility, providing up to 28 hours of care per week per resident, assisting with ADLs (with a 1-person assist), and providing all 3 meals per day. Residents must be able to bear weight on their feet to do a pivot transfer. Hospice services can come in for residents who enroll. If a resident's needs go beyond our scope of practice, they will need to move to a higher level of care. We strive to keep residents here through the end of their lives but depending on their care needs, they might have to move. A limited number of apartments are still available.

For residents who request or require some assistance with their daily living activities, Wilson Commons’ Assisted Living apartment homes provide privacy, comfort, and personal care for each resident.

Our lifestyle amenities include free scheduled transportation to nearby shopping, restaurants, grocery stores, outdoor areas for personal gardening and enjoyment, short and long walking trails, full service salon, fitness room, library, weekly banking services, Chapel with Catholic Mass and Lutheran Service, and a calendar of fun social and recreational activities.
