Overview on the types of professionals on our staff and the training they undergo at our facility

Our staff includes RNs, LPNs, PT/OT, caregivers, and multiple department leaders. The type of training they undergo depends on their department, education, ongoing education needs related to their certifications, and the requirements by the Illinois Department of Public Health. At minimum, our staff undergo 8 hours of senior-centered training as part of orientation and then an additional 8 hours of dementia training.

Measures to ensure the safety and well-being of our residents

Our community is secured from 8pm to 8am, and all residents are provided with an emergency pendant to call for help in case of an emergency. We create care plans to address more specific safety needs of each resident, taking an individualized approach to care given the varied needs of each senior.

Resources or support services to aid residents' transition into the community

Our community hosts a Welcoming Committee (run by residents) to assist with transitioning to a new home. Additionally, our move-in coordinator provides check-ins with the adjusting senior for the first 2 weeks of their stay.

Transportation services for residents

Yes, we offer transportation services for residents.

Dining experience

Our meals are served restaurant-style in our dining room. We host a "Special of the Day" in addition to a standard menu, and are happy to accommodate allergies and alternative diets.

Process for addressing resident complaints or issues

If a resident has a complaint, we encourage them to speak directly with our Executive Director. He will then assign the issue to the appropriate department for investigation and follow-up with the resident and family. There is also a form that the resident may fill out.

Cost of living in our community

Assisted Living ranges from $4500-$6400 depending on apartment size and care needs.

Policy on communication with resident's families

Resident's care plans are updated at change in condition and/or annually. We communicate any changes to the family during this time. In addition to this formal communication, residents and their families have access to our Director of Nursing or Assistant Director of Nursing whenever they have questions or would like a general update.

Short-term or respite stays

We offer respite stay in Assisted Living for $275 a day.

Pet-friendly policy

Our Independent Living is pet-friendly. Pets are allowed for Assisted Living on a case-by-case basis.

Waitlist status

No, we do not currently have a waitlist.

Vaccinations in the community

Residents are able to get Covid and flu vaccinations in the community, generally in the fall.

Aging in place

Yes, residents may age in place in our community.

Living Options

Within the community, we host 3 different types of living – all under 1 roof. Independent, Assisted and Memory Care options are available. Each apartment is private, with senior-friendly amenities like walk-in showers, grab bars and emergency pullcords.

On-site Services

Our 24-hour on-site staff provide a variety of services to our residents. Meals, a robust social calendar (check us out on Facebook!), housekeeping and more make Willow Falls feel like home!
