Overview of Professional Staff and Training

Community First is a diverse organization that includes a 42-bed short-term rehab facility, an AL/Memory Care facility, and 2 continuing care communities. We employ 3 Licensed Nursing Home Administrators (LNHA) and each facility is equipped with a Director of Nursing and MDS, Quality Control, and Infection Prevention nurses.

Safety and Well-being Measures

As my role is in IL Sales, I am not an expert in this area. However, we have measures in place to ensure the safety and well-being of our residents.

Transition Resources and Support Services

We offer a Resident Services and Resident Lifestyle team, in addition to resident ambassadors, to aid residents' transition into the community.

Transportation Services

Yes, we offer transportation services for residents, but there is a charge based on mileage.

Dining Experience

We have Resident Dining Rooms at all campuses, and Morris Food Service is our culinary partner. Additionally, we have a restaurant open to the public at our Berkeley Square campus.

Resident Complaints and Issues

Residents typically approach Resident Services or, if more clinical in nature, the Director of Nursing or Administrator to address complaints or issues.

Cost of Living

AL ranges from $132-$225/day. IL entrance fees range from $90k-over $400k with monthly fees ranging from $2,000-$3,500/month for services.

Communication with Residents' Families

Our Social Services Coordinator is responsible for care conferences, and nurses call families with changes in care plan/medical conditions.

Short-term or Respite Stays

Yes, we offer short-term or respite stays at $240/day.
