Our Staff

Our staff consists of a Medical Director. His name is Dr. James Shane.

We have a Nurse Practitioner who assists Dr. Shane.

Our Director of Nursing is Karen Miller.

We have 4 charge nurses (2 at each nurse’s station).

We have CNAs and CMAs.

Safety Measures

The measures we take to ensure our residents' safety include resident checks every 2 hours minimum. We do not have a secure unit at our facility, but we do have staff that work up front who monitor the doors during business hours. Each staff member is required to take safety modules regarding safety, responding to emergencies, handling biohazards, etc.

Social Work Services

We have a Social Worker in our building who can assist with the transition back to the community or a different SNF or ALF. She works diligently with our residents to provide them with the necessary resources needed for the transition, whether it be medical equipment or finding the right Home Health or Hospice company.

Transportation

Our facility does have a bus that provides transportation for our residents. Transportation is typically used only for resident appointments, but transportation can be provided on a case-by-case basis.

Dining Options

Our residents have a few options for dining. CNAs will ask our residents about their meal preferences. Our cook works closely with the dietitian to ensure our residents get the best nutrition possible. Residents have the choice to eat in the dining area with other residents or, if they choose to stay in their room, their meals will be brought to them.

Handling Complaints

For any complaints that residents may have, they can notify any staff member. It is our duty to ensure that those complaints are managed properly to ensure that our residents have an enjoyable experience. If the complaint is major, such as suspected abuse, neglect, etc., we immediately report it to the state for investigation.

Care Plan Meetings

Our social worker schedules care plan meetings to go over the care that the resident receives, any concerns, likes, dislikes, etc. Any family members listed on the resident contact sheet can call the facility to speak with the nurses to inquire about the resident.

Short Term and Respite Stays

We do offer short-term and respite stays. If the patient is on hospice and would like to have a respite stay, the hospice company can reach out to us and provide us with the proper documents to review for the respite stay. If all checks out, we can provide the respite stay. If no hospice company is involved, then we would request notes from the patient’s PCP to review. Once we get the clinical approval, we can get with the Business Office Manager to discuss prices. Typically, our private suite is $319/day, and a semi-private room is $236/day.

Pet Policy

Although pets are not allowed to stay while the patient is in our care, we do allow pets to visit with their human. The only requirement is that the pet has a current vaccination card that will need to be provided upon entering the facility. A copy will be made and put in the patient’s file.

Vaccinations

COVID and Flu vaccinations are provided to our residents. Staff will reach out to resident representatives for consent. If the patient can consent, then they can complete the form themselves.

Aging in Place

Residents are allowed to age in place. We are a Long-Term Care facility, and many of our residents are on hospice services here as well.
