Rate Increase Disclosure

Our history of rental increases for the past Three (3) years. Rate increases due to change of level of care based on the assessment of individual’s needs &/or increase on over all operational costs.

Year 2021 - rate increased average of $50 - $100/month or ≈ 2.5%

Year 2022 - rate increased average of $50 - $100/month or ≈ 2.5%

Year 2023 - rate increased average of $100 - $250/month or ≈ 5-6.0%

Our staff are initially trained with quarterly and annual sessions as required by the State.

Measures to Ensure the Safety and Well-being of Residents

Residents are checked at least every 2 hours and as necessary. Building and gate settings are in place and secured since the facility is licensed for Dementia.

Resources and Support Services for Transition into the Community

Yes, we provide known sources and services to help transition to community, medical, and non-medical facilities.

Transportation Services for Residents

Medical transportation is provided within a 7-mile radius but without a companion.

Dining Experience

Our spacious dining room is set up with staff serving the meals. Alternate foods are always available.

Process for Addressing Resident Complaints or Issues

The facility has an open-door policy regarding complaints or issues. They can be reported verbally or in writing directly to the Administrator or their designee.

Policy on Communication with Residents' Families

The Administrator or designee informs and/or advises the family or responsible party of any concerns, issues, updates, etc., via phone or email at any given time. Information or reports are collected as deemed necessary, e.g., food intake percentage.

Short-term or Respite Stays

No, we do not provide Respite Stay.

Pet Policy

No, the facility does not allow residents to keep pets, but they are welcome to visit and provide therapy to our residents with safety. The pet owner will be responsible for the safety of the pet and other residents in the event of any accident caused by the pet, regardless of whether it is intentional or unintentional.

Waitlist

Yes, we currently have a waitlist.

Covid and Flu Vaccinations

Yes, if covered by health insurance, an outside provider will administer on-site (providers usually allow with a minimum number of individuals).

Aging in Place

Yes, as long as it is within the scope of our license. The facility has no bedridden clearance unless the resident is under hospice services.

Facility Licensing and Guidelines

Our facility is licensed for Dementia with Delayed Egress and operates according to the guidelines of both State and Federal Rules, covered with blanket restrictions where applicable.

Our facility is also a provider of Medi-Cal Assisted Living Waiver Program, Calpers Insurance & Anthem Blue Cross (subject to authorization).
