Private Pay Bed Hold Rates

• Private Room Daily Rate: $380.00
• Semi-Private Room Daily Rate: $370.00
• 3-4 Bed Suite: $360.00

Medicare/Medicaid and most other insurance companies do not pay to hold a bed during a Resident's temporary absence from the facility. Under certain circumstances, Medicaid may pay to hold a bed; please refer to the facility Medicaid Bed Hold Policy that is included in the admission contract.

Beauty/Barber Services

• Hair cut: $15.00
• Shampoo & Set: $15.00
• Permanent: $50.00
• Color: $40.00

Additional listing of services and fees available upon request

Guest Meal: $5.00

Cable TV: Provided free of charge

Medications: Prescription medications are not included in your Medicaid room and board rate and will be billed separately by Omni Care Pharmacy.

Physician visits, Laboratory, X-ray, Podiatry, Vision and Dental Services: These services are offered at our facility and are billed separately by the service provider. Please review service provider's consent forms signed upon admission.

Therapy

• Oral/Speech Pathology, (per visit): $180.00
• Physical/Occupational Therapy (each 15 minute modality): $60.00

*Medically necessary physical, occupational and speech therapy will be billed to Medicare and your supplemental insurance company.

Private pay room rate includes routine nursing services as defined in the Admission Contract.

Private Pay Bed Hold Rates

Bed hold rates are the same as Private Pay daily room rates as noted above.

Oxygen

• Concentrator & Supplies: $65.00
• E-Cylinder Tanks: $9.00
