Assisted Living

Assisted Living at Wesleyan Village offers a range of services to assist residents to live in the comfort of their apartment. Support is available to assist residents in maintaining their health, safety, and dignity. Residents are assessed by a nurse prior to residency. The village offers a tiered plan in assisted living to meet resident’s individual needs. Residents who require additional services beyond level IV, based on a nurse assessment and at the discretion of the Village interdisciplinary team, may purchase additional services from a licensed and bonded community home care agency of their choice. Services beyond the limitations that can be provided in the residential care facility/assisted living will necessitate a transfer within the continuum of care at Wesleyan Village or to a facility of choice. The following rates are applicable to Reeder, Wesley, Boyd and Smith.

• Scheduling of appointments/diagnostic testing (Includes all necessary paperwork needed by doctors and reporting communication with doctors)
• Emergency Services (Assessments, call to 911 and/or family and paperwork required by the emergency room)
• Completed assessment score of 0 to 2

Level I • Level I service + $938

• Completed assessment score of 3-14

Level II • Level II service + $1524

• Completed assessment score of 15-25

Level III • Level III service + $1990

• Completed assessment score of 26-36

Level IV • Level IV service + $2461

• Completed assessment score of 37 or more

All of our Assisted Living apartments include the following:

• 3 meals per day
• Weekly housekeeping
• Laundry (Reeder, Boyd, Wesley residents only)
• All utilities including basic cable, local phone and Wifi
• Call Pendant
• Weekly scheduled shopping trips
• 24/7 access to Nurses and STNA staff
