Availability and Waitlist

We have a waitlist for our Medicaid beds due to limited availability. However, we strive to accommodate residents in our assisted living or skilled nursing units as needed, without a current waitlist for these services.

Respite and Short-term Stays

Wellbrook South Bend offers respite care for up to 30 days in both our assisted living and skilled nursing units, providing temporary relief for caregivers.

Pet Policy

Pets are welcome in our assisted living units. We understand the importance of pets to our residents and offer provisions for their stay, including a deposit and a monthly fee.

Transportation Services

Our community provides a wheelchair-accessible van or bus for outings and appointments. We organize outings twice a week and offer transportation to physician appointments three days a week.

Our Staff

Our dedicated team includes RNs, LPNs, QMAs, and CNAs, ensuring comprehensive care for our residents. We also have a medical director who oversees patient care, with physicians available daily.

Safety Measures

For the safety of our residents, we have a call light system in place. Residents can also use personal pendants for additional security.

We keep families informed through a monthly newsletter, an activities calendar, and our presence on social media platforms like Facebook and Instagram. Our website also serves as a resource for information.

Addressing Concerns

Wellbrook South Bend has an open-door policy for complaints and concerns. Residents and their families can approach any staff member, department head, or the executive director to express their concerns.
