Level of care is determined by a comprehensive assessment in which we assess an individual’s needs, abilities, and physical and mental condition. The assessment is based on current diagnostic, medical and social history obtained from the individual’s primary health care provider and any other third party that provides health care services. The process also includes a face-to-face assessment with the prospective resident and the individual’s legal representative (if any) and family members to determine what the individual views as their needs, abilities, interests, and expectations.

Payment Options

We only ask for 1 year of private pay before Medicaid enrollment as well. 😊
