Individualized Service Plan

The charges for additional assistance with Activities of Daily Living are based on the total minutes of assistance needed by the resident. The Individualized Service Plan is billed in 15-minute increments allowing us to charge for ONLY the services you require.

Resident Assistant: $12 per 15 mins

Registered Nurse: $27 per 15 mins

Memory Care Levels of Care

Memory Care residents will have a maximum service rate of $1,200. This would be in addition to the monthly rent. These services are based on the individualized service plan based on the assessment by a nurse and time spent with your loved one

Level 1: $625 (6-10.99 hours per week)

Level 2: $900 (11-15.99 hours per week)

Level 3: $1,200 (16+ hours per week)

Assessments will be conducted by a nurse on pre-move, 30 days, 90 days, annually and/or with any change of condition. This assessment determines the Individualized Service Plan for the resident.

A’la Carte

Residents may choose to add on services each month such as guest meals, tray service, garage rental, additional laundry service, private transportation, RN services, etc.

Lifestyle Fee

A one-time fee of $2,500 will be due at move-in to cover upkeep, replacement of carpet or apartment repair and general maintenance.

Respite Stays

Our respite program is designed for individuals who need supervision and/or services for a short period of time, typically a minimum of five days and a maximum of 30 days. The respite services include: three restaurant style meals per day; housekeeping; laundry; transportation; and assistance with Activities of Daily Living. Keelson Harbour offers this program to those who meet our move-in criteria. This service is ideal for those who have a caregiver who needs to go out of town, a spouse in the hospital, someone who needs additional recuperation time from a hospital stay, or simply to determine if assisted living is the right decision for an individual on a long-term basis. Our respite suite is beautifully furnished and has all the comforts of home.

Overview on Types of Professionals and Training

We have 2 Registered Nurses (RNs) on duty Monday through Friday during business hours. Additionally, we have a triage of nurses available for call at night that can be accessed by our Resident Assistants (RAs). We maintain at least 2 RAs on duty 24 hours a day.

Safety and Well-being Measures

We ensure the safety and well-being of our residents with 24-hour staff availability. Each resident receives a pendant, which, although not officially classified as an "emergency pendant," can be used for emergency situations. These pendants are primarily used for help with bathroom breaks or general assistance.

Resources and Support Services for Transition

We offer support services for transitioning into the community, including an Alzheimer's support group for memory care residents that meets on the last Tuesday of each month.

Transportation Services

We provide a bus system that takes residents to their medical appointments twice a week in town and three times a week out of town (up to Worthington MN). The bus also takes residents to the YMCA twice a week and to Fareway, Hy-Vee, or Wal-Mart every other week. Additionally, residents often use the Rides system, which is not associated with us but offers low-cost personal rides throughout the week.

Dining Experience

Our dining experience includes a daily menu that changes regularly. We also offer a general menu that residents can order from throughout the day if they do not prefer the meal of the day.

Complaint and Issue Resolution Process

Our director handles most complaints. Residents often start their complaints with me when they first arrive, but the director manages the resolution process. If issues persist, we provide a list of phone numbers for state or Ombudsman contacts to ensure residents feel heard and addressed.

Communication with Resident's Families

Communication with residents' families is a priority. Families usually call me for updates on their loved ones. We place a magnet on the fridge with my phone number, as well as the contact information for the director, maintenance, nurses, and activities coordinator.

Pet-Friendly Policy

Yes, we are a pet-friendly community with a $1,000 fee and a monthly charge of $50.

Waitlist Information

Currently, our memory care has a waitlist, as well as rooms with a patio.

Vaccinations

We offer Covid and flu vaccinations within the community.

Aging in Place

Residents are able to age in place as long as we receive the necessary permissions from the state, especially for those on respite. We do not have a nursing home on-site but work closely with one approximately four blocks away (Accura).
