Types of Professionals on Our Staff and Their Training
All of our communities are skilled nursing facilities, which means there is always a Registered Nurse on-site 24/7. We also employ Licensed Practical Nurses and Certified Nurse Aides. All of our nursing staff are required to complete continued education, and we are always offering specialized trainings for them as well to better serve our patients.
Measures to Ensure the Safety and Well-being of Residents
Every single staff member is a mandatory reporter, and we are all held accountable by the state health department and regulations. Each facility is also assigned an ombudsman, which is a third-party advocate for Residents.
Resources or Support Services for Transitioning into the Community
Yes, there is a program through Medicaid called Transitions. This program is designed to help Residents transition back to the community after being in a facility for an extended period of time. We have social workers that will assist through this process.
Transportation Services for Residents
Yes, we do have our own transportation, and if they are booked up, we will arrange alternative transportation.
Dining Experience
Residents have the right to either receive a meal tray for their room or go to the dining room to enjoy their meal there. They are given a couple of options per meal, and there is also an always offered menu if nothing is sounding good.
Process for Addressing Resident Complaints or Issues
Our communities have a complaint card system where Residents or loved ones are able to submit a complaint that way. All of our staff members have an open-door policy, and you all should be able to voice concerns to any staff member as well. Those concerns get diverted to the Administrator, and they will follow up to help resolve the situation.
Cost of Living in Our Community
Each community has a different cost, but skilled nursing costs roughly $10,000/month under private pay.
Policy on Communication with Resident's Families
Each community has a social worker, who will most likely be your main point of contact. They will arrange a care conference upon admission to the community to discuss specific expectations for communication.
Short-term or Respite Stays
Yes, our communities do offer respite stays, and those are either covered by Medicaid or under private pay. Under private pay, the cost would be calculated by day, as our communities do have daily rates instead of monthly rates.
Pet-Friendly Policy
Pets are allowed to come visit as long as we have a copy of their vaccination records. Pets are typically not allowed to stay overnight.
Current Waitlist Status
Some of our communities do have a waitlist, but most do not and have bed availability now.

Yes, this can be arranged either in-house or staff can assist in setting up an outside appointment.
