Overview on the Types of Professionals on Our Staff and Their Training

Our professionals are hired with their certified credentials. This includes Certified Nursing Assistants, Certified Medication Technicians, Licensed Nurses (RN & LPNs), a Certified Culinary Director, and a Resident Lifestyle Director who is certified as well. These team members provide direct assistance within their respective fields to the residents. Any additional training includes basic COGIR orientation regarding our values of care, individualized training, and the routine State-mandated requirements of care.

Measures to Ensure the Safety and Well-being of Our Residents

This is a very broad question. I would like to know your specific concerns to address them completely. The safety measures for those in Memory Care differ from those for our Assisted Living residents. Generally, however, we have the front desk staffed from 8am-8pm. During these hours, the doors are secured, and all residents must sign in and out when coming and going from the community. All outings are done with additional team members to provide extra support and supervision.

Resources and Support Services to Aid Residents' Transition into the Community

We utilize several resources. First, our Creative Memoirs form is our “Get to Know You” form, which we ask you to complete with your loved one. This information is important for orienting our team members to your loved one, ensuring a smooth and special transition. Our rehab teams conduct safety evaluations of resident apartments, so they feel safe moving around. We have Counterpoint Geri-Psych services to assist residents with the anxiety that sometimes comes with this type of transition. We also utilize our ambassador committee to help new residents adjust during this difficult time.

Transportation Services for Residents

We do offer transportation services. We transport residents to local grocery stores, Target/Walmart, etc. during shopping outings. Doctor appointments are currently within a 5-mile radius to the local physician practice in the Melford Towne Center.

Dining Experience

Our Encore dining program allows our residents to dine in our beautiful dining room, which boasts a restaurant-style dining atmosphere. Residents order from our menu, which includes an “Always available” option. Our servers assist residents with selecting an offering if needed. We manage the basic specialty diets and help residents select items best suited for their specific needs.

Process for Addressing Resident Complaints or Issues

We are all here to serve our residents. Therefore, a complaint is viewed as a concern affecting the service our resident is receiving, not as a complaint. Our goal is to provide the best service. A resident can share their concern with the concierge (who will get it to the right Director to address), or they can ask to see the Director over the area of concern. We all have open-door policies and welcome opportunities to talk with our residents, even if they are voicing a concern. I am usually made aware and will follow up if there is an issue getting the concern addressed. If the resident prefers, they can complete a grievance form, and the issue will be addressed the same way.

Cost of Living in Our Community

The cost includes rent (based on the apartment style selected), the level of care assistance if needed, and a one-time community fee.

Policy on Communication with Residents' Families

Community update letters are sent to discuss basics within the community. We also keep families updated via our Loop program, sharing upcoming activities and events in advance. As for communication about your loved one, you are always welcome to call our nurse to inquire. However, if something happens or creates a clinical concern, our nurse will reach out to you. We also have care plan meetings so you have a chance to discuss concerns and/or progress.

Short-term or Respite Stays

Yes, we do offer 30-day short term stays. The cost varies based on the size of the apartment and the clinical assistance required. Respite suites start at $6500 per month, all-inclusive.

Pet-Friendly Community

Yes, we are a pet-friendly community. The resident needs to be able to take care of their fur family independently.

Current Waitlist Status

Yes, we have a waiting list for several apartment styles, in Revere Memory Care and Assisted Living.

Vaccinations for Residents

Absolutely! We offer our Fall Flu/Covid clinic typically in October.

Aging in Place

Yes, this is their home. We offer Palliative Care, Hospice/Care and Comfort options. Aging in place here is based upon the end of life wishes your loved one expresses.
