Our pricing starts at $4,470 for a studio and $4,970 for a one bedroom.

Medicaid Waiver

We also accept a Medicaid waiver and depending on your loved one's income, we would take a portion of it for their portion of the rent.

Overview of Our Staff and Their Training

At our nursing facility, we take pride in having a diverse team of highly trained professionals dedicated to providing the highest quality of care to our residents. Here’s an overview of the types of professionals on our staff and the training they undergo:

Registered Nurses (RNs)

Our RNs are responsible for overseeing the medical care of our residents, including administering medications, monitoring vital signs, and developing care plans. They undergo rigorous training, including obtaining a nursing degree and passing the NCLEX-RN exam. In addition, they participate in ongoing education and training to stay updated on best practices and new developments in patient care.

Licensed Practical Nurses (LPNs)

LPNs provide essential support to our RNs by assisting with basic nursing care, such as wound care, collecting samples for testing, and monitoring residents’ health. They complete a practical nursing program and must pass the NCLEX-PN exam. LPNs at our facility also receive regular in-service training on various topics related to patient care and safety.

Certified Nursing Assistants (CNAs)

CNAs are crucial in assisting residents with daily activities such as bathing, dressing, and eating. They complete a state-approved training program and must pass a competency exam. We ensure that our CNAs receive continuous training to enhance their skills in providing compassionate and respectful care to our residents.

Third-Party Physical, Occupational, and Speech Therapists

These therapists work with residents to improve mobility, regain daily living skills, and enhance communication abilities. They hold relevant degrees and certifications in their fields and receive ongoing training to stay current with therapeutic techniques and approaches.

Dietitian and Nutritionists

These professionals are responsible for developing and monitoring individualized nutrition plans for our residents, ensuring they meet their dietary needs. They hold degrees in nutrition and dietetics and are trained in providing medical nutrition therapy.

Activities Coordinators

Our activities coordinators plan and facilitate recreational and social activities that promote engagement and well-being. They have backgrounds in recreation, therapy, or related fields and receive training in activity planning and resident interaction.

Housekeeping and Maintenance Staff

These team members ensure that our facility is clean, safe, and comfortable for our residents. They undergo training in sanitation, infection control, and safety protocols.

All staff members participate in regular training sessions, including emergency preparedness, infection control, dementia care, and resident rights, to ensure they are well-equipped to meet the needs of our residents. Resources for Residents' Transition into the Community

Yes, we allow hospice to get involved.

Transportation Services

Yes, we provide local transportation to doctors' appointments for our residents in Assisted Living with Supportive Memory Care. If the resident would like to go shopping, we ask the family to assist them with outings like that. Please see attached file for our transportation schedule.

Dining Experience

We provide three meals a day. Our residents always have a "this or that" option and get their choice of drink. Please see attached for our weekly menu.

Complaint and Issue Resolution Process

They fill out a grievance form to whom they would like to speak with or whatever they may need fixed.

Communication with Residents' Families

Yes, our nursing staff will always inform families of any changes. We also reach out monthly to see if families have any concerns they would like to address.

Short-term or Respite Stays

Yes, we offer respite stays. Respite stays are a minimum of 30 days. For the Terrance, Assisted Living with Supportive Memory Care, the cost would be $4,470. This includes levels of care, three meals a day, light housekeeping laundry, local transportation to doctors' appointments, and activities for our residents to enjoy! We also provide basic cable and wifi.

Pet-Friendly Policy

Yes, our residents are welcome to have a pet if they are capable of caring for the animal.

Covid and Flu Vaccinations

Yes, we can do COVID and flu vaccinations.

Aging in Place

Yes, and often we have hospice get involved to assist with end-of-life care. I do not have a waiting list and have 5 studios available at this time.

About Towne Centre

Towne Centre accepts Medicaid Waiver!

Those who are on a fixed income and need assistance can apply for the Medicaid Waiver for Assisted Living and Memory Care at Northwest Indiana Community Action (219) 794-1829.

Towne Centre is also Private Pay with competitive rates in Northwest Indiana to help individuals spend down while applying for the Medicaid Waiver.

Assisted Living and Memory Care

Towne Centre is an Assisted Living community and provides Memory Care for individuals with Alzheimer’s and Dementia.

Services and Amenities

Towne Centre has 24-hour nursing, 3 restaurant style meals daily, light housekeeping and laundry weekly, local transportation and activities tailored to our residents’ needs and interests.

Floor Plans and Facilities

Towne Centre has several assisted living and memory care floor plans to choose from. All apartments come with individual climate control units, an emergency response system, cable TV and Wi-Fi access. Our Assisted Living apartments come with a full-size kitchenette, which includes an oven/stove and a full-size refrigerator.

Additional Benefits

Towne Centre also features other convenient benefits such as: Contracted Doctor for our residents, On-Site Hospice Services, a beauty shop for men and women, outpatient physical therapy, a well-stocked library, a chapel for religious services, a movie theater, and a gift shop.
