Qualified Medication Personnel (QMAPs)

• We have QMAPs and Care staff.
• We partner with home health and Hospice.
• All of our staff undergo dementia training.

Individualized Resident Care

• We care for each resident to their own individual needs.
• We have a care conference with each family to make sure we can meet the needs of each resident.

Support Group

• We are going through training to offer a support group for Alzheimer's Disease (ALZ).
• This will help with all aspects of dementia.

Transportation

• We offer transportation for outings and Doctor appointments.

Dining Experience

• We have a home-like dining experience from preparing the meal to dining together as a family.

Welcome Packet

• We give each family a welcome packet with residents' rights and house rules.
• The packet includes a copy of a grievance form and procedure, and information about their Ombudsman.

Communication

• Our communication is different for each resident.
• Some families want to be contacted at different times, and we strive to meet these needs.

Respite Stay and Rent

• Yes, we offer respite stay and our rent is month to month.
• It depends on availability and the size of the room.

Pet Friendly

• We are pet friendly and have several pets in both of our communities.
• The residents and staff love them.

• We do have FLU and COVID clinics in our communities.

Age in Place

• We are honored to provide age in place in our homes.
• Yes, we do. We have 1 private small or 1 large private room available.
• We are an all-inclusive community.
