Do you guys currently have a waitlist?
Yes, we have a waiting list for long term care. Things change daily so I do not have a time frame for a bed opening at this time.
Could you share pricing and any other general information?
$275 per day for a semi-private room and $350 per day for a private room.
Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?
We have many professionals i.e RN's, CNA's, LPN's, Physical Occupational and Speech therapist, Administrative team, Social Services Director and assistant, Dietician, Wound Care Nurse, Medical Director and Medical DR's that follow, Nurse Practitioner and Physician assistants. State regulated training that is ongoing throughout the year.
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
We have state and federal regulations, licenses, and inspections that are required. We do have a wander guard system in place for residents that are in need.
Do you offer any resources or support services to aid residents' transition into the community?
We have a social services team that can assist.
Do you offer transportation services for residents?
We can assist with setting up transportation and do have a van that can accommodate for approved transportation.
Could you tell me a little about your dining experience?
Since COVID we have provided meals in the residents' rooms. This is a Nursing home/ Skilled nursing facility so our dining is not typical like assisted living communities.
What is the process that takes place if a resident has a complaint or an issue they want addressed?
We have a policy in place such as a grievance policy. Any complaints or issues will go through the social services department and we will investigate and address.
What is your policy on communication with resident's families?
We have a care plan meeting upon admission and throughout your stay with different departments and if you would like an update, you can call and speak to the department needed.
How do you keep them updated about their loved ones' well-being?
You receive a letter in the mail with your care plan date which provides an update and you can call for additional information.
Do you all offer short-term or respite stays? How much does this cost?
Yes, we do provide respite/Short term stays. The pay is listed above and is required up front. If patient is coming from the hospital Short term rehab is provided by your health insurance.
Are you all a pet-friendly community?
Yes, we are pet friendly for Visitation only. All shots must be update to date a copy provided.
Are residents able to get Covid vaccination and flu vaccinations in the community?
Yes.
Are residents able to age in place?
Yes.
