Be advised that the Level of Care charges as outlined below are the current rates for individuals not currently receiving Elderly Waiver assistance. Therefore, be advised that the current private pay rates listed below would become effective if you no longer qualify and/or meet the criteria to remain an Elderly Waiver (EW) recipient and were to become a private pay resident.

Senior Living Packages:

Each resident is required to purchase one of the following care:

• Level One $1,455.00 per month 0 points
• Level Two $2,045.00 per month 1-15 points
• Level Three $3,155.00 per month 16-30 points
• Level Four $4,075.00 per month 31-45 points
• Level Five $4,825.00 per month 46-60 points
• Level Six $5,575.00 per month 61-75 points
• Level Seven $6,325.00 per month 76-90 points
• Level Eight $7,075.00 per month 91-105 points

Points are determined based upon need per nurse assessment. Higher care level packages are available as needed.

Included in all Assisted Living Packages:

• Weekly light housekeeping
• Initial and subsequent service plan updates
• Emergency pendants and/or pull cords
• Clinical record maintenance
• Registered Nurse 40 hours per week and on-call triage 24 hours per day
• Ongoing monitoring by nursing staff
• Additional nursing hours may be available on-site during select awake hours
• Personal care staff on duty 24 hours per day
• Two safety checks per day
• Two loads of laundry per week
• Monthly vital sign checks
• Care coordination
