Staff Training and Certification

All the staff in our community are trained to work with residents with Dementia or Alzheimer’s. Phoenix provides special training for our staff. Our care staff are Certified Nursing Assistants. Our Med Techs are all state Certified Medication Aides. We have an LPN on staff as well.

Safety Measures

Our safety measures are designed specifically for residents with Dementia or Alzheimer’s. We have 2 wings in our community, each designed to help those with memory decline. There are locking doors for those at risk for wandering to ensure their safety. The door to each room is locked from the outside to ensure each resident's belongings and sleep are not disturbed by others. We aim to ensure all of our residents are safe and well cared for.

Support and Activities

Upon arrival, our staff ensure residents receive everything they need. A key part of our support system is our activities program, designed to keep residents engaged throughout the day to aid in their transition. We encourage residents to interact and participate as much as possible. We also offer a monthly support group for families to help with stress and understanding the disease more.

Transportation Services

We offer transportation for our residents with advanced notice to ensure an authorized driver is available. A family member must meet us at the destination. The first 15 miles are included in our cost, with a surcharge applied thereafter.

Dining

Our dining services include 3 home-cooked meals a day and snacks throughout the day, with seasonal menus that change every 3 months, all included in our cost.

Grievance Process

If a resident or family has a complaint, we have a Grievance Process in place. The process starts with filing a complaint with our executive director, who ensures every complaint is taken seriously and handled properly.

Room Options and Pricing

We offer different room options, with private rooms at $5,395 and companion rooms at $4,295. The cost of care levels, starting at $560, is determined by an assessment conducted by our nurse, based on a point system.

Communication with Families

It is our requirement to notify families of any incidents, such as falls or wounds. We also communicate via email for updates on illnesses and restrictions due to state guidelines.

Residency Terms

We currently do not offer short-term or respite stays and do not allow animals to stay in our community. However, families can bring pets with a vaccination record on file. Support animals are brought in by our hospice companies.

Vaccinations

Our residents can receive the covid and flu vaccines provided by our contracted pharmacy once a year. Families are notified and responsible for signing their loved one up for the clinic.

Community Features

Our community is a stand-alone memory care facility that allows residents to age in place. We offer various amenities including podiatry, dental services, primary care, and therapy services on-site, as well as salon services every Wednesday.

About Us

We are a stand-alone memory care community. We specialize in Alzheimer’s and Dementia. We provide all meals for our residents as well as hydration. Our residents have a full activities program that is based off the 6 dimensions of wellness.

Healthcare and Wellness Services

We offer an on-site doctor, podiatry every 3 months, dentist every 6 months as well as on-site therapy services. We have a full-service hair salon for our residents.

Accommodation Options

We have 2 options for rooms. We have a private room and a companion room. It is your choice of which you prefer for your mom.
