Our desire is to serve and partner with you and your family in an effective, caring, and supportive environment. We offer four different rates for your convenience and are here to help you find the financial assistance available to you to make this transition as smooth and as affordable as possible. MONTHLY RATES

Private Room with Private Bathroom: $3,478

Private Room with Community Bathroom: $3,260

Semi-Private Room with Private Bathroom: $3,043

Semi-Private Room with Community Bathroom: $2,826

Please have the following documents available for the admission process:

• A copy of the last two bank statements for all your accounts
• A copy of your benefit letter showing your benefit amounts from Social Security and/or SSI/SSP
• A copy of a document showing your pension benefit if applicable
• Photo ID
• Social Security Card
• Birth Certificate
• Medicaid, Medicare, and all health insurance cards
