Overview of Staff and Training
Our staff consists of LPNs, Medication Technicians, CNA's, and Resident Assistants. They undergo initial training once onboarded at our community and continuing education requirements to ensure their skills are staying sharp. We are partnered with our pharmacy which provides skills checks for our medication technicians and training classes for fundamentals of providing care and specialized dementia training. We also have a trainer whose main duty is to ensure that staff are receiving training and continuing education. As our community is specialized in Memory Care only, we have further staff training requirements to complete to ensure we are meeting the needs of our residents effectively.
Safety and Well-being Measures
Our community is secured via a keypad lock. We encourage freedom of movement for our residents while allowing them to be in a safe and secured environment. We also provide 2-hour checks throughout the day and night for our residents although during the day, they are mainly out doing activities as we do encourage that. We have several residents that utilize bed alarms at night which will alert our staff if they are trying to get up. To ensure the well-being of our residents we will also re-assess residents in a timely manner to ensure that if their care needs fluctuate that we are monitoring that and informing our staff of the changes in the care plan that we create upon the initial assessment.
Transition Support Services
Our team is hands-on and supportive during the transition of a resident moving into our community. We take the move day by day and allow the resident to adjust to their routine at our community at their own pace. We also provide resources for families as we have a support group that meets monthly at our community hosted by the Alzheimer's Association.
Transportation Services
Yes, we do offer transportation for our residents on Tuesdays and Thursdays within a 10-mile radius.
Dining Experience
Our residents are offered healthy choices during meals. The meals are served restaurant-style. We have 4 neighborhoods where our residents reside and each neighborhood has its own dining room. This allows for a more comfortable feel for our residents and also allows for more eyes on our residents while they eat and ensuring they are eating well and drinking proper amounts of fluids. Our chefs are also trained to provide special doctor-ordered diets like puree, chopped, etc. We offer a healthy and balanced diet made from fresh ingredients.
Resident Complaints and Issues
We take this very seriously and want to ensure that any issue is addressed. Our families receive contact information for all of our leadership team and may contact any one of our leadership team members with a concern and we will work together to ensure that it is addressed timely and effectively.
Communication with Resident's Families
We understand the importance of communication and prioritize that for our families. We have several key practices in place to ensure proper communication: Our nursing team will directly call the families with updates on how their loved ones are doing and with any changes in conditions that we notice. Additionally, we utilize an app called "Sagely" that our families can log onto on their phone and see what activities their loved ones have participated in each day. We also have a primary care physician group that comes in weekly and they use an online portal for families called "Same Page" to view notes from the doctor.
Short-term or Respite Stays
We offer respite stays at $265/day. We typically have a 30-day minimum stay.
Pet-Friendly Community
We do allow pets to visit with loved ones as it can be therapeutic for an individual with Alzheimer's/Dementia.
Covid and Flu Vaccinations
Yes, we set up vaccine clinics through our pharmacy and will inform the families when the upcoming clinic will take place at our community so that they can fill out the consent form for their loved ones to get a Covid/flu vaccination.
Aging in Place
Yes, we are truly an age in place community where families can have hospice services come into our building.
Our pricing is all-inclusive meaning it includes the care plan, medication management, 3 meals per day, daily snacks, housekeeping, laundry, daily activities tailored to memory care needs, cable and utilities. Additionally, we have a highly qualified physician that serves as Medical Director, licensed nursing supervision 24 hours per day, and unmatched staff-to-resident ratios to ensure personalized attention. At The Memory Center, we believe the challenging conditions of an aging brain should be embraced with a caring, interactive community designed around the individual. We do not have a wait list at the moment but do have limited availability in our private apartments.
