DAILY LEVEL OF CARE RATES

The level of care is based on a comprehensive assessment required by the Centers for Medicare and Medicaid Services (CMS) called the Minimum Data Set (MDS). This assessment indicates how much assistance is required with activities of daily living based on medical needs. Assessments are completed for each resident on admission, on a quarterly basis, and/or with a change of condition.

ANCILLARY CHARGE LIST

Routine ancillaries, supplies, and equipment provided to all residents are included in the individual base Room & Board and Level of Care. Facility reserves the right to charge for an additional ancillary charge when any individualized specialty supply, equipment, or nursing treatments are required due to personal preference or a Doctor/Medical Specialist order.

TRANSPORTATION CHARGES

Family is encouraged to transport the resident to and from appointments. Facility vehicles are available for transporting residents if family is unable to make other arrangements. Please allow us to schedule any appointments that involve the use of facility vehicles.

MEDICARE PART A SERVICES

The Manor at Elfindale is a Medicare certified Skilled Nursing Facility. If you have Medicare Part A, it may cover your stay up to 100 days if you meet the qualifying criteria. Days 1 – 20 are paid in full by Medicare and days 21- 100 have a daily copayment amount established Medicare. The Medicare co-payment amount is $204.00 per day for 2024. The copayment and any non-covered services will be your responsibility. Medicare A qualifying criteria are:

  • A. Consecutive 3-night acute hospital stay within 30 days of admission
  • B. Required daily skilled service such as therapy 5 days per week or a nursing service 7 days per week
  • C. Physician certification of need for skilled care
  • D. Treatment must be for the same condition as the hospital stay

MEDICARE PART B SERVICES

Medicare B may cover therapy services, some urological supplies, wound care dressings, enteral feeding supplies, and therapy supplies. Medicare Part B will pay 80% of these items or services and 20% is considered coinsurance (copayment). We have made arrangements to bill those services when medically necessary and you will only be billed any remaining balance after Medicare Part B and any supplemental insurance has paid.

INSURANCE

The Manor at Elfindale participates in Medicare Advantage plans and other primary insurance plans whenever possible. If a resident qualifies for Medicare Advantage or a primary insurance coverage, covered services and supplies will be billed to those policies on your behalf. We do not accept assignment of insurance benefits unless we are a contracted provider. Any charges not covered by insurance are the resident’s responsibility.

Overview of Our Professional Staff and Training

Our staff includes a diverse range of professionals such as RN, LPN, CMT, CNA, administration, housekeeping, laundry, maintenance, and culinary staff. They undergo continuous education throughout the year to meet state regulations and guidelines. This includes training on abuse and neglect, dementia, skills fairs, and generalized education for continuum of care.

Safety and Well-being Measures

To ensure the safety and well-being of our residents, our facility is equipped with a fire panel that triggers alarms in case of fire, smoke, flood, or any other emergency. The facility doors are locked and require a keypad code for entry, which is provided to staff and families of residents. Additionally, we have security personnel who patrol the campus during the evening and overnight.

Support Services for Residents' Transition

When a resident is discharged to the community, we assist in services based on their request and needs to facilitate their transition.

Transportation Services

Transportation is available on a scheduled basis through coordination with our transportation coordinator, subject to her schedule and availability.

Dining Experience

We offer open dining in a central location, serving breakfast, lunch, and dinner. Residents have the option to dine in their room if they prefer.

Addressing Complaints and Issues

If a resident has a complaint or an issue, they are encouraged to contact our grievance officer in social services. The necessary steps to resolve the issue are initiated and followed up on in a timely manner.

Communication with Residents' Families

Upon admission, each resident completes a HIPPA form that specifies which family members can receive clinical information. Care plan meetings are held within a week of admission and every three months thereafter. Family members can contact or visit a nurse at any time and request visits with the physician on a scheduled basis.

Short-term and Respite Stays

We do not offer respite stays. Our services include short-term rehabilitation from a hospital setting, as well as long-term care or memory support long-term care.

Pet-Friendly Community

Yes, we are a pet-friendly community on a visiting basis. Animals are not allowed to stay with a resident but are welcome to visit.

Covid and Flu Vaccinations

Yes, residents are able to get Covid and flu vaccinations within the community.

Aging in Place

Yes, as a CCRC, we offer independent living, assisted living, and long-term care facilities on the same campus but in different buildings. The Manor currently has a waiting list.
