Do you guys currently have a waitlist?
No waiting list at this time.
Could you share pricing and any other general information?
Our semiprivate room rate is $320/day, our private room rate is $400/day.
Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?
We currently have RNs, LPNs, and STNAs on staff, as well as our contracted vendors like ViaQuest for psych services, ancillary services, a MD w/his own staff of Nurse practitioners. We also contract with Continuum therapy for PT, OT, and ST services, and a dietician to manage weight loss/gain. The training varies by department as far as the amount of time they are in training, but we always ensure the staff member is confident and comfortable prior to coming out of training.
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
We have 24-hour staffing, as well as a wonder guard system, an alarm system for when patients are getting out of bed, and consistent room checks.
Do you offer any resources or support services to aid residents' transition into the community?
Yes, our SW assists with all transition needs back to the community which includes home health services, follow up appointments, PCP assistance, and any equipment needs.
Do you offer transportation services for residents?
Yes, we have a facility bus that we can use to transport patients, if they cannot go by WC we have contracted with several transport companies that we could arrange stretcher transport if needed.
Could you tell me a little about your dining experience?
We offer fine dining in our dining room, or patients are able to eat in their rooms where they can get a tray delivered.
What is the process that takes place if a resident has a complaint or an issue they want addressed?
We have a concern form that we use to address all concerns or complaints that could arise. These forms are given to administrator who then assigns an appropriate department head to address the concerns.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
We offer an initial care conference upon admission to drill down expectations, emergency contacts, plans of care, and discharge plans. From that meeting, we set a plan and as things progress, we can arrange updated care conferences every other week, monthly, or quarterly. This is all determined by preference of family, patient and facility.
Do you all offer short-term or respite stays? How much does this cost?
We offer both respite and short term stays. We bill everything through insurance first before anything is billed to patient. Cost would be room and board which is listed above, plus medications, and any therapy services not covered by insurance.
Are you all a pet-friendly community?
We are pet friendly for visitation, not to live in. Pets must be up to date with shot records.
Are residents able to get Covid vaccination and flu vaccinations in the community?
Yes, we offer both vaccines.
Are residents able to age in place?
Yes.
