• Personal Care 1 Bedroom: $7,150
• 2nd Person Fee: $2,750
• Memory Care: $7,950 all-inclusive

Includes apartment

• 3 chef prepared meals a day
• Verbal cuing with personal care & activities of daily living
• Medication management
• Trained & caring staff 24/7
• Weekly housekeeping
• All utilities including phone, cable, and internet
• Coordination of all healthcare needs
• On-site physicians & accessibility to on-site clinical labs
• Programming that promotes 8 dimensions of wellness including physical, social and religious

LEVEL 2 • $495

Include all services in Level 1 PLUS

• Assistance with dressing

Base Pricing & Level Guide

ADDITIONAL PRICING AS FOLLOWS:

• Deposit: $1,000 (non-refundable)
• Community fee: $2,500 (non-refundable, one-time fee)

Respite Care:

• Personal Care: $215 per day (+ Level), for first 30 days $240 per day (+ Level), days 31+
• Memory Care: $265 per day, for first 30 days $300 per day, days 31+

Transportation:

• Round-trip - $30 (flat fee, unaccompanied) $17 per half hour for staff accompanied

Other Services:

• Alert Pendant: $195 replacement fee if lost or broken
• Personal Laundry Services: provided on-site: $120 per month
• Pet Security Deposit (non-refundable): $500 (some restrictions apply, see administrator)
• Furniture Rental: $100 per day
• New Furnished Room: $2,700 one-time fee (chair, dresser, nightstand, mirror, bed frame, head board and low-profile mattress)
• Disposal of Personal Property post-discharge: $750
