Overview on the Types of Professionals and Training

Our director of resident services is a Licensed Practical Nurse (LPN) who has been with the company for over 10 years. The director of memory care (Bridge to Rediscovery) has also been here for over 10 years and is currently in nursing school. All of our caregivers and medication technicians are certified through the state of Arizona. We conduct monthly Continuing Education Units (CEUs) for the entire staff.

Measures to Ensure Safety and Well-being of Residents

Every resident has an individualized care plan that is updated with family input every 3 months in memory care and every 6 months in assisted living. If there is a change of condition in between, we will meet as a team to revise the plan so that well-being and safety are always addressed.

Resources or Support Services for Transition into the Community

We partner with many healthcare professionals depending on the person's needs. Additionally, we try to match new residents with a long-term resident. A day or two after a new resident moves in, I personally visit them to go over the activity calendar and discuss the available activities. My assistant and I encourage all residents to participate in activities of their interest.

Process for Addressing Complaints or Issues

Any team manager is happy to assist with concerns, but ultimately, I am responsible for addressing any complaints or issues a resident might have.

Cost of Living and Respite Stays

We offer short-term or respite stays if available. The cost is $7,000.00 per month with a one-month minimum, which includes care and medications.

Communication with Resident's Families

Many of our family members visit regularly and are considered part of our extended family. Powers of Attorney (POAs) are notified immediately if there is an incident or change in condition.

Pet-Friendly Policy

Yes, we are a pet-friendly community. Pets must be under 30 pounds, and there is a one-time pet fee of $500.00. The resident must be able to care for the pet. We also have pet therapy and a community cat.

Waitlist Status

As of now, we do not have a waitlist. We currently have openings in our memory care unit.

Vaccination Availability

Residents are able to get Covid vaccinations and flu vaccinations within the community. We host vaccine clinics regularly.

Aging in Place

Yes, residents are able to age in place in our community.
