Monthly Basic Service Provided

Basic Rate: $4,783 month (1st Person)
Second Person: $1,578.00 month
The Basic Rate includes the following Basic Services:

Meals
Three meals daily served restaurant style, in our lovely dining room (Selective menu, includes preparation and supervision of special diets, does not include room service)
Laundry
Weekly Laundry Service (Personal clothing, linens, normal washing/drying and changing bed linens)
Weekly Housekeeping
Vacuuming, dusting, Cleaning and disinfecting bathroom, kitchenette area and trash removal.
Staffing
Two Resident Care Assistants are generally on duty during the day - In addition to the Resident Administrator and one nurse. In the evening and night one Resident Care Assistant is on duty.
Activities
The Gardens provides daily activities to stimulate physical and mental capabilities. The Gardens encourages residents to participate in social, recreational, and leisure activities so that resident's lives may be more meaningful. Activities also assist residents in attaining their optimal social, physical, and emotional well being including self-care and maintenance.
Basic Personal Assistance
Meal reminders, monthly blood pressure and weight checks with facility nurse. Assistance scheduling healthcare appointments.

Staffing Schedule
We have an STNA that works 6:30am-6:30pm, another STNA works 7am-2pm, then an STNA works 4-9pm to help get through dinner and assist residents to bed. Then at 6:30pm-6:30am we have an STNA that comes in.
I am the full-time day shift nurse that works in the assisted living. I’m an LPN and a certified director of assisted living, also I have CPR and am a certified infection preventionist. Overnight, we have an LPN/RN on duty that oversees the assisted living.
We have a continuing education software system called Relias and the STNAs have first aid training. Our STNAs have taken med pass classes, they have the ability to pass routine medications with the exception of Narcotics and insulin. Those have to be administered by nursing.
Rounding Procedures
We have rounding procedures put in place to check on residents throughout the day and night shift. Our front doors lock at 9pm. We do not have personal alarms/bed pad alarms in AL.
Staff and Resident Support
Staff is wonderful and have longevity and help residents adjust into their new home. We do have psych/counseling services available if needed.
Transportation
We do have a bus for transportations to and from appointments (a fee is involved).
Dining Services
Our dining room serves restaurant style, residents have a choice at meal times and if they don’t like the options alternatives are always available. If they do not come to the dining room they have the option to eat meals in their rooms.
Resident Council and Communication
We have a resident council that is held and residents can verbalize likes and dislikes. Also, I address any complaints/concerns in real time.
I communicate or the other nurses with families if there are any changes in conditions, new orders, lab results, etc.
Respite Care
We do offer respite care. I will attach the pricing list, which is broken down into segments depending on the level of care needs.
Pet Policy
We are a pet-friendly facility but the resident has to be able to take care of the pet themselves, including feeding, letting the dog out, and emptying litter boxes, etc.
Vaccinations
We do offer Covid/flu and pneumonia vaccines.
Resident Transition and Aging in Place
Residents can age in place here, they have to continue to be a one-person assist. If they are a two-person assist/transfer, they don’t qualify to stay in regular assisted living. They can reside in the dementia unit AL being a two-person transfer. We are also private pay so when residents run out of money they can transition into the nursing home where they accept Medicaid.
We do have a small waiting list to get into assisted living.
