Overview of Professionals and Their Training

Almost all staff are licensed (RNs, LVNs, CNAs, CMAs, LNFA, LMSW, LBSW, PTs, PTA, OTs, COTAs, SLP) prior to hire, but we provide extensive staff education on a monthly, weekly, and annual basis.

Safety and Well-being of Residents

We do not have a secure unit but operate 2 units staffed appropriately 24/7, complying with all Life Safety regulations for Skilled Nursing Facilities.

Support Services for Transition into the Community

We have a psychologist who visits weekly and offer activities 7 days a week.

Transportation Services

Yes, we offer transportation services for residents.

Dining Experience

We serve 3 meals per day in our dining room and 4 snacks on the units. Many residents choose to take their meals in their rooms. Many residents are NPO and have tube feedings, both continuous and bolus.

Complaint and Issue Resolution Process

We have a formal grievance process managed by the Administrator and Social Workers.

Communication with Residents' Families

We chart by exception, and communication generally happens via calls. We also conduct Quarterly Care Plan Meetings to keep families updated about their loved ones' well-being.

Short-term and Respite Stays

Yes, we offer short-term or respite stays at the same daily rates.

Pet-Friendly Policy

Pets can visit if shot records are provided.

Covid and Flu Vaccinations

Yes, residents are able to get Covid and flu vaccinations in the community.

Aging in Place

Yes, residents are able to age in place. For Medicaid Pending, we do have a waiting list.
