Welcome to Our 32 Bed Personal Care Facility

We are a 32 bed personal care facility where our staff is available 24 hours a day to provide assistance with care. We offer assistance with grooming, dressing, bathing, medications, meals, laundry, and more. Our facility is designed to ensure that all our residents receive the care and support they need in a comfortable and welcoming environment.

Full Service Beauty Salon & Chaplain Services

Our full service beauty salon is available every Tuesday & Friday on our 2nd floor, providing an additional comfort of home. This service is a separate charge on the monthly rent bill. We also provide chaplain services, short-term rehabilitation services, and spacious community rooms for our residents to enjoy.

Room Features

Our rooms offer a range of amenities including a telephone outlet with optional in-house service, individual closets, room climate control, cable TV, an in-room emergency call system, and a private full bathroom with an emergency call system. A personal telephone in the room is available for a $20 monthly charge.

Admissions & Staff Certifications

We are currently accepting admissions and have all room types available at this time, with no wait list. Our Personal Care Assistants (PCAs) are medication administration certified, diabetic certified, and CPR/FIRST AID/AED certified. Our staff undergoes yearly training on DHS requirements including HIPPA, Abuse/neglect, fire training, dementia, and more. I am the nurse (LPN) of the facility and am on call when not on-site.

Safety Measures & Transportation

Our facility is not a “secure unit” but we take measures for the safety and well-being of our residents. Our stairway doors are alarmed, and our main entrance is locked after 8pm and unlocked at 8am. We check on the residents every 2 hours, though in practice, this happens about every 30 minutes. We do not offer transportation for our residents but have a facility van for activities in the community.

Dining & Complaint Resolution

We have a beautiful dining room on the 1st floor where residents come for meals. Water, coffee, juice, and snacks are always available. If a resident has a complaint, they are directed to me as the administrator to assist with putting the complaint in writing. Complaints are investigated and a resolution is made within 3 business days.

Communication with Families & Pet-Friendly Community

We over communicate with families regarding any decline in a resident's health, new orders from physicians, falls, etc. We are a pet-friendly community and contract through a pharmacy that administers all COVID-19 and flu vaccines. Our residents are able to age in place, with hospice services available through a hospice agency that comes into our facility if needed.

Hospice Services & Long Term Care

If a resident has a decline and we can still meet their needs safely, we offer hospice services. However, if we cannot safely meet the resident's needs, we are attached to a long-term care facility and the resident would be assessed for transfer to their side. We currently do not have a wait list.
