Overview of Professionals and Training

We employ a diverse range of professionals including LPNs, RNs, Certified Nursing Assistants, Occupational/Physical/Speech Therapists, PTAs, COTAs, and Social Workers. Additionally, we collaborate with Northern Lights for a Nurse Practitioner and our lead physician/medical director. Safety and Well-being Measures

Our facility is equipped with a call light system in all suites, and staff utilize voceras for efficient communication. We also maintain a semi-secured memory care neighborhood to ensure the safety and well-being of our residents.

Transition Support Services

We offer various resources to aid residents' transition into our community, including resident council, social services support, psychologist/psychiatrist services, and a Community Life Services department that provides engaging activities tailored to individual needs.

Transportation Services

Trillium Woods Independent Living offers transportation services for its residents. In our Health Center, we use external medical transport companies for residents' appointments, with costs varying based on distance.

Dining Experience

Our dining experience features flexible dining times, varied menu options, and restaurant-style service with fine china and linens. We accommodate special diets including dairy-free, low sodium, and altered texture diets.

Complaint Resolution Process

Residents, family, or staff can fill out grievance/complaint forms, which are reviewed by our social worker, director of nursing, and/or administrator for resolution.

Cost of Living

The cost of living in our Health Center/skilled nursing varies between $390-$540 per day, depending on care needs. Exact rates are determined upon admission.

Family Communication Policy

We conduct care conferences and provide a patient portal for families to stay informed about their loved ones' well-being. Our staff is also available for updates at any time.

Short-term and Respite Stays

We offer short-term and respite stays, with costs dependent on the specific insurance plan. A $4000 deposit is required for respite stays, with an average rate of $400 per day.

Pet-Friendly Community

Our community welcomes pets with up-to-date vaccinations and required veterinary documents.

Waitlist Information

We have a 4+ year waitlist for long-term skilled nursing care, with priority placement for our Independent Living residents. There is no waitlist for short-term or respite care.

Vaccination Services

We provide Covid, flu, and pneumonia vaccinations to our residents.

Aging in Place

Residents can age in place, transitioning from Independent Living to more assisted living within our community as needed.
