Additional person: $1,575 per month plus applicable level of care fee.

A one-time Resident Administrative Fee (non-refundable) of $3,300 per resident is due at lease signing.

All Care Levels are determined by an individualized assessment and personalized plan of care.

Tall Oaks does not charge a separate medication management fee. These rates are subject to change until signing of the lease prior to occupancy.

Overview of Staff Professionals and Training

Our staff who have been with Tall Oaks for between 8 and 28 years enable us to provide excellent quality of care and outcomes. We provide constant training through our program called Relias Learning. Additionally, we offer off-site training by sending our directors and staff to our home office and leading age locations.

Safety and Well-being Measures

We have a Concierge on staff from 7 am to 8 pm. Our exterior doors lock at 8 pm, though we allow visiting hours after 8 pm for residents on hospice with pre-arranged notice. We have an alert system on our Memory Care floor to ensure the safety of those with memory impairments. Additionally, round-the-clock staff are present on all floors, and our directors make after-hours visits.

Resources for Residents' Transition

We provide resources and support services tied into the resident’s care plan, which is created before they arrive.

Transportation Services

Transportation is provided within a 5-mile radius of the community.

Dining Experience

We offer a restaurant-style dining room with two specials each meal, and residents are served just like in a restaurant.

Process for Addressing Complaints

Complaints are addressed by either our Executive Director or Director of Nursing, depending on the nature of the issue.

Communication with Residents' Families

Communication preferences vary among families. Any time there is a change in the care level, we schedule a care plan meeting with the family. We also hold at least one care plan meeting annually.

Short-term or Respite Stays

Our respite stays start at $5,500/month. For more details, please see the attached respite brochure.

Pet Policy

We are a pet-friendly community with a one-time fee of $150.

Vaccinations

Residents are able to get both Covid and flu vaccinations in the community.

Aging in Place

Residents are able to age in place. We offer an enhanced level of care and have memory care available if the need arises.
