Daily Rates and Payment Information

Our daily rate for semi-private rooms is $485. We do have a (very) few private rooms and the daily rate for those is $500. Payment for a full 30 days is required on admission day unless other arrangements have been made with our business office.

Short-Term/Respite Stays

Short-Term/Respite Stays are on a case-by-case basis and the admission process is the same as it would be for Long-Term Care placement.

Welcome to Our Skilled Nursing Facility

We are a small, 80-bed Skilled Nursing Facility staffed by dedicated RNs, LPNs, and CNAs. Our team also includes an on-site Social Worker to assist with the transition process and coordinate care plan meetings, involving input from nursing, therapy, dietary, activities, and social work departments. We are committed to addressing concerns and answering questions to ensure a smooth resolution to any issues.

Nutrition and Hydration

Our facility provides balanced nutrition and hydration through meals and multiple snacks throughout the day. A Dietician works closely with our Medical Director, NP, and nursing staff to create individual meal, nutritional, and weight goals tailored to each resident's specific dietary needs and health conditions.

Transportation and Visits

We coordinate transportation for medical appointments but not for social outings. Therapy may assist with car transfer training when appropriate. Pets are welcome to visit, provided they are up to date on vaccines, with records provided for the Department of Health.

Admission Process

Upon admission, we identify the main emergency contact and require documentation of a Healthcare Proxy (HCP) and Power of Attorney (POA) for residents unable to make decisions for themselves. Our Infection Prevention Nurse reviews vaccination status and arranges for necessary vaccinations.

Bed Availability and Rates

Bed availability for Long-Term Care can change weekly, and we are often at full capacity. Our daily rate for semi-private rooms is $485, and for private rooms, it's $500. Payment for a full 30 days is required on admission day unless other arrangements have been made.

Admission for Long-Term Care

For placement in a Skilled Nursing Facility in New York State, a current PRI (Patient Review Instrument) and Screen, completed by a certified nurse, are required. We review these documents carefully to ensure we can meet the needs of the individual seeking placement. Tours can be arranged, and if a bed is offered and accepted, we plan for admission usually within 24-48 hours.
