We have (1) floor plan immediately available, and it is a 1-bedroom, 555 sq ft apartment that ranges from $3499 - $4100/month.
Included in the Monthly Rate
- Dinner (two starters, entree, and dessert) every night.
- Housekeeping every-other week.
- Linens and towels washed weekly.
- All utilities, cable + internet.
- All activities and fitness programs!
- Transportation:
- Monday/Wednesday/Friday - anywhere within a 20-mile radius!
- Tuesdays are our designated grocery trip day!
- Thursdays, we do group trips/ outings.
- In-house physical, occupational, and speech therapy. (This is considered "out-patient rehab" and is billed through Medicare part B!)
- 24/7 response staff in the building.
- All apartment maintenance.

I apologize for the delay. I wanted to make sure I had adequate time to reply to this email, thoroughly!
Could you provide an overview on the types of professionals on your staff and the training they undergo at your facility?
I attached a PDF to this email titled "Move in Momentum Chesterton" that introduces all of our directors. This newsletter is included in every new resident's welcome folder! StoryPoint as an organization prides itself on a rigorous hiring process and continued education. Different departments have different durations, but all of them start their training off-site at a different StoryPoint before they start at their permanent community. This is to demonstrate a successful example of their future department and allow for hands-on training.
Speaking specifically to our Wellness Department:
- Our Nurse Practitioner is in the community every Tuesday. (She can be established as someone's primary care provider, but it is more common for her to act as a liaison between our resident's and their established primary care provider).
- Our Wellness Director is in the building M/W/F 7 - 4 and T/TH 12 - 8. She oversees all of our Wellness Department and all aspects of establishing, scheduling, and communicating care. Her background is in physical therapy.
- Our 24/7 care staff are CNA/ RA-grade caregivers.
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
Safety, security, and a sense of belonging are the pillars of our community and company. This mantra is so deeply ingrained in our culture, I don't know where to start! The attached PDFs labeled "StoryPoint Handbook" + "August Calendar" will speak to the specifics of safety and well-being within our community.
Do you offer any resources or support services to aid residents' transition into the community?
Included in the experience is an orientation with all of our directors within the first week of moving in. We also have a Welcome Committee comprised of 12 residents. One of the Welcome Committee members is assigned as a "buddy" to the new resident and will offer additional guidance and support in the acclimation process.
We also offer a "Welcome Escort" care package. It is $475 for 30 days of unlimited escorts from a caregiver. A lot of our residents have utilized this service and really appreciate the support.
What is the process that takes place if a resident has a complaint or an issue they want addressed?
The process would start by bringing it to the attention of our Executive Director. How it is addressed depends on the issue itself. Our Executive Directors lead the experience our residents have each day. From ensuring employees are fulfilled and doing their best work to forming deep relationships with every resident, everything our executive directors do is with the experience of our residents in mind.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
I am not sure what you mean by "policy" but we have very high expectations for communication.
Do you all offer short-term or respite stays? How much does this cost?
Yes, we have fully furnished 1-bedroom apartments. Our minimum length of stay is 30 days at $5910. Our maximum length of stay is 90 days and any duration over 30 days would be $155/ night.
Are you all a pet-friendly community?
Yes! Two pets per apartment, no larger than 30lbs/ each, and there is a $35/mo pet fee.
Are residents able to get Covid vaccination and flu vaccinations in the community?
Yes. Quarterly we offer a vaccine clinic in the community.
Are residents able to age in place?
Yes, we strive to ensure all of our residents have an opportunity to age in place. Only in these instances would StoryPoint not be able to meet someone's needs:
- If someone became an elopement risk, endangering themselves.
- If someone became aggressive and/or violent, endangering themselves or others.
- If someone's care exceeds the capacity of senior living and they need skilled nursing (what a nursing home provides). There is a waitlist for everything else!
