2023 Assisted Living Rate Worksheet – The Vista

Nursing Care Services:

Based on preadmission, quarterly assessment or change in condition the following monthly charges may apply.

Physical function: Note: Please know additional needs like total dependence, increased behaviors, medical complexity, or assist of two or more people will likely trigger additional conversations on transfer to skilled care or 3rd party assistance.

• Scores between 1 – 20 $0
• Scores between 21 – 35 $200
• Scores between 36 – 55 $500
• Scores between 56 – 75 $1,000
• Score 76 and above or Category B $2,500
• Incontinence assistance $500
• Oxygen assistance $150
• Medication administration $600
• SJU Pharmacy Medication/OTC Bubble Packing Included
• Non-SJU Pharmacy Medication/OTC Bubble Packing $3/med/month

Cognitive function: Note: If at risk for additional wandering or behaviors, alternative housing arrangements may be necessary.

• Scores between 0 – 3 (MMSE >25) $0
• Scores between 4 – 7 (MMSE 24 – 15) $400
• Scores 8 and above (MMSE <15) $600

Services/procedures: If required or requested to be performed (nebulizer treatments, vitals/weights performed more than monthly, wound care/ointments, insulin administration, etc.), the following rate applies; these are determined by physician’s orders. Note: Residents are responsible for the cost of specialty dressings for wound care.

• 2 – 7 Nursing Contacts per Month $200
• 8 – 30 Nursing Contacts per Month $400
• More than 31 Nursing Contacts per Month $600

Additional Charges (if applicable):

• Personal Laundry Services $100/single/month $150/couple/month
• False Fire Alarm (charged by city) $250/occurrence
• Re-admission if medically out of the community for greater than 48 hours. $250 One Charge per each Re-admission
• IT Tech Support $50/hour Per Request

One Time Service Fees:

• Administrative Admission Fee $500/person $500/person
• Apt. Security Deposit (Refundable minus any damages) $2,000
• Pet Deposit (Non-refundable) $750 Limit 2 pets

Please note: If procedures and care levels are beyond the ability of our staff or the State’s allowed parameters, you will be notified to discuss options for meeting the care required by the resident inclusive of alternative housing arrangements. This rate worksheet should not be construed as an individual’s appropriate level of care for our community. Our residents have a level of priority access to St. John’s (subject to qualifications).
