Overview on Staff Professionals and Training

We have licensed CNA's, LPNs, & RNs for our care staff. All nurses have skills checks that are completed yearly to maintain compliance for their license. All care staff also undergo monthly Inservice topics, as well as any other topics as needed. On our clinical management team, we have more LPN's, RN's (director of nursing), and an IPN (infection prevention nurse) who undergoes training with CDC and Local Health Department. On our administrative/support team, we have LNHA (administrator), CDM (certified dietary manager) who is also Serve Safe Certified. All kitchen staff are also required to have current food worker cards in compliance with Washington State regulations. We have an in-house RD (registered dietitian) as well. Therapy/Rehab professionals include in-house speech language pathologists, occupational therapists, and physical therapists, as well as PTA's and COTA's. All staff/departments undergo scheduled monthly Inservices and more as needed.

Measures to Ensure Resident Safety and Well-being

We have 24/7 care staff in the facility. Our call light system includes emergency calls in restrooms and common areas. We have a wonder guard system in place to aid patients who are an elopement risk. This system automatically locks exterior doors when the patient is within range. Visitors must sign in at the front reception desk, which is the only way in and out of the building as all other doors remain locked. Individualized interventions are reviewed and implemented as needed.

Resources and Support Services for Transition into the Community

Our Social Services team collaborates with our clinical team as well as the patient to make the transition back to the community as safe as possible, which may include assistance with local programs or Home Health services.

Transportation Services

We have a facility van, although most patients utilize local WTA paratransit within the community to go on outings or to medical appointments.

Dining Experience

Residents can dine in our large dining room or have the option of dining in their room. The kitchen runs on a 4-week menu cycle with varying breakfast options, two entree/meal choices for lunch, and two choices for dinner. Additionally, a variety of always-available options such as daily soups, grilled cheese, hamburger & fries, hot dog & fries/chips, chef salad, fruit plate, deli/BLT sandwich, and more are provided. Residents are interviewed for preferences upon admission and as needed. The kitchen accommodates specialty breakfasts like omelets and breakfast sandwiches.

Process for Addressing Resident Complaints or Issues

We have grievance forms and team communication forms that residents or staff can fill out on behalf of a resident. Concerns are addressed as soon as they are made known to staff.

Policy on Communication with Residents' Families

Upon admission, patients fill out a contact list to determine who can receive information. Changes in condition, lab work, etc., are reported to the designated contacts. Many residents choose to be their own responsible party.

Short-term or Respite Stays

We offer respite stays. If a patient does not have insurance that covers respite stay, they must pay privately. Payment must be made 30 days in advance for patients admitting under private pay. Unused days/money will be reimbursed following discharge. The cost for a private room is $12,000 a month/$400 a day, and a semi-private/shared room is $11,250 a month/$375 a day.

Pet-friendly Policy

We allow pets to visit with a record of vaccination, but pets are not allowed to stay overnight. Patients must fully care for their animals, including vet appointments, grooming, and daily care, without assistance from facility staff.

Covid and Flu Vaccination Availability

We offer COVID vaccines and annual flu vaccinations in our facility.

Ability to Age in Place

Yes, when living in a SNF for LTC, we provide various care needs and offer more assistance as the patient ages and their conditions change. Currently, we have long-term care beds available, as well as respite and rehab beds. Shared and private rooms are available, with private rooms tending to be at private pay rates and shared rooms available as private pay or Medicaid.
