RATE SHEET: ST. CLARA’S REHAB & SENIOR CARE
LINCOLN, ILLINOIS
Effective: May 19, 2023 for new residents
July 1, 2023 for current residents
BASE RATE (per person per day):

The above charges include room and board and basic nursing care.
The bed-hold rate is equal to the daily base room rate.
Arrangements can be made for temporary/respite care, not to exceed 14 consecutive days. This service
offers nursing care and support services when primary caregivers are unable, absent, or in need of temporary
reprieve of caregiving duties. The rate is fifteen dollars ($15.00) per day added to the base rate quoted
above.
ADDITIONAL SERVICES:
Additional Charges - All Rooms
Nursing Supplies and Equipment: Nursing supplies are built into the cost of the base room rate except for
oxygen, oxygen concentrators and resident specific specialized equipment which will be billed monthly.
Other Charges: Beauty shop, barber charges, transportation services, guest meals, and personal purchases
are billed monthly (contact facility for services provided), as used, unless facility requires pre-payment of
services. Contact facility for details. Physical therapy and/or other therapy ordered by the Doctor will be
billed monthly. Doctor fees and prescription drugs are billed directly by the supplier.
Contract Rights: All the rights and responsibilities of a resident pass to the resident’s guardian, next of kin
or sponsoring agency or agencies if the resident is adjudicated incompetent under State law or is determined
by his/her physician to be incapable of understanding his/her rights and responsibilities.

Professionalism and Culture
We should always maintain professionalism as a part of our culture here. Since the facility has multiple departments, we have different requirements and training for each. Our nursing department is the largest one, and our nurses have obtained a license through the state of Illinois for RN or LPN. Similarly, our CNAs have all passed the required testing through the state. To keep our staff up to date with their educational requirements and/or required hands-on training for the State of Illinois every month via Relias.
Safety Measures
Measures in place to ensure the safety of our residents include staff on-site 24/7 and door code access to enter and leave the facility. We also have a built-in system throughout the facility that is tested for accuracy monthly for residents who need to wear wonder guards to ensure their safety. We follow laws and regulations related to the Life Safety Code and have an emergency disaster manual available to staff with policies and procedures listed and approved by the fire chief here in Lincoln. We also do monthly training with staff for fire drills.
Social Service and Admissions
As the Social Service Director and Admissions Coordinator, I am here to help residents who are transitioning back into the community and their families by providing them with the necessary resources and services. Some examples of the services I can arrange for them include home health services, medical equipment, PT/OT/ST therapies if needed, meals on wheels, or transportation.
Transportation and Dining
We provide transportation services for facility residents. Any fees are discussed during admission paperwork.
Residents have various dining options available to them. They can eat in the dining room with other residents, in the common areas on the hall, or enjoy private dining with their family in our private dining area. While some residents may prefer to eat in their room, we always encourage them to participate in social activities and come out for some socialization. We offer their choice of what’s on the menu, and if they would like something else, we always have an “anytime menu” available.
Complaints and Grievances
Should a resident have a complaint or concern that needs to be addressed, they have the right to file a complaint/grievance with me. Once they do, I address it with whichever department is needed, devise a solution to correct the issue, and follow up to ensure it is resolved.
Communication Policies
We have policies and procedures in place for communication with the Resident and Authorized persons to speak to regarding the resident. Upon admission, we complete an HIM (Authorization to speak to) in which the resident and/or POA give us the authorization to discuss anything related to the resident, which can be changed at any time. If there is a change in condition, new medication orders, imaging results, and so forth, we will notify the PCP and POA of all. Communication is key.
Respite Stays and Pet Policy
We offer respite stays for up to 14 days at a time at the room rate + 15.00/day, depending on whether they request a private or semi-private room.
We are a pet-friendly community here at St. Clara’s. We have families who always bring in their pets to see their loved ones. Our Activities Department has emotional support dogs come in as well to visit.
Vaccinations and Aging in Place
Residents can receive COVID and flu vaccines here in-house or if they choose to go out in the community. We have an IP Nurse on staff who follows up with vaccines after admission when we review the consent. We also offer Hepatitis B, Pneumonia, RSV, and shingles vaccines in-house.
Lastly, yes, residents can age in place here. That said, we have our sister facility next door, Castle Manor, which is Supportive living. Should a resident be more appropriate there because they do not need skilled services or long-term care and do not feel like the community is best, we encourage the transition. Then, should they decline or need more assistance, we are here to offer care all the way up to the aid of end-of-life care. We do currently have a waiting list.
