Assessment and Care Levels

Each level is individually assessed and set by the D.O.N. and resident, services can slightly vary from the services listed below at the discretion of the facility based on the varying needs for each individual.

Level I

Completely independent, may include limited verbal cues

Level I w/Meds

Independent but requires assistance with medication. There is a medication charge in addition to the basic monthly rate at this care level - $250.00

Level II

• Assistance with bathing, dressing, and grooming; verbal or stand by or limited physical
• Minimal assistance with toileting and eating; verbal or minimal physical
• Supervision of, or minimal assistance with medication administration.
• Additional nursing services (e.g., simple dressing changes, frequent monitoring of medical status, etc.)
• And additional services as reviewed and deemed appropriate by D.O.N.

Level III

• Assistance with ambulation and transfer, stand by or limited physical
• Incontinent care; not including bowel care when consistent
• Assistance with eating; limited
• Administration of medications
• Additional nursing services (e.g., complex and/or multiple dressing changes, on-going assessment and monitoring of treatments, daily monitoring of medical status, etc.)
• And additional services as reviewed and deemed appropriate by D.O.N.

MEMORY CARE services are an all-inclusive and individually assessed as each one of our accommodated memory care community members are unique in their needs and we will work with them and their loved ones to assure customized comforts committed to assuring their stay and life are at their best quality!
