Private & Companion Monthly Rental Rates
Sonata Assisted Living & Memory Care offers several affordable month-to-month rental rates coinciding with your private or companion suite floor plan in either a Harmony Assisted Living or Serenades Select Memory Care neighborhood. Monthly rates include a variety of services and amenities including utilities, laundry service, and meals. For a complete list of amenities, please view our enclosed Services & Amenities sheet.

Personal Care Monthly Pricing: One Rate Does Not Fit All
Care is one of the most important services we provide, and most residents require basic care, assistance, and supervision. To address each resident’s individual needs, we offer up to five levels of care in Assisted Living and nine levels of care in Memory Care.

Prices and floor plans may vary based on community location and availability. A one-time, non-refundable Community Fee will apply to each full-time resident. Community Fees fund the maintenance of shared areas within the community, as well as capital improvements.

PERSONAL LEVEL OF CARE

Level of care is determined after an assessment is performed by a licensed nurse and AHCA Form 1823 is completed by the primary care physician. Assessments are completed prior to admission, at admission, and again 30 days after move in. Thereafter, assessments are completed every 6 months or if there is a change of condition. Level of care rates are monthly.
