Community Details
This detailed information was received through an email from the Community Relations Director at Sommerset Neighborhood via email on 2024-05-15.
Professional Staff
Professional staff are licensed and/or certified with years of experience, and over 10-15 years of longevity here at Sommerset for many of us. All staff are required to go through several hours of continued education and training on a quarterly basis.
Security Measures
All doors are armed with a delayed egress alarm, and there are door codes that resident family members are given at the time that their loved one moves in. All guests are required to check in using a kiosk upon arrival and are given a name tag to be worn while in the community, as an additional safety precaution for our residents.
New Resident Introduction
Upon move-in of a new resident, they are introduced to management as well as the direct caregivers in the area in which the residents will be moving into. The Activities department welcomes the resident, also giving them a monthly activity calendar, inviting the resident to the next activity, and offering to introduce the resident to other residents to help make the transition smoother and less overwhelming.
Transportation
We have a company SUV used for limited transportation, such as scheduled doctor’s appointments and/or activity trips. All doctor’s appointments must be scheduled in advance, and the resident must be able to transfer in and out of the vehicle safely and stay at the appointment alone or with a responsible party who can call the community for pick-up.
Dining
All residents are encouraged to eat in the dining room with other residents, which helps them not only eat better but also get to know other residents on a personal level. This assists in the transition and adjustment to their new living arrangement. A dietician creates our weekly menus; residents have a choice between the meal(s) of the day or can order off an 'A la carte' menu.
Resident and Family Communication
At the time of move-in, residents and their family members are given a list of manager names and titles, as well as the opportunity to meet them personally. They are encouraged to reach out to the manager of the appropriate department for any complaints. Our Administrator, Jessica, is also available to residents if needed.
During the pre-admission process, names and contact information are collected and written on the resident demographic sheet in the front of each resident’s medical record. Family members listed to receive information and updates regarding their loved one’s care are called as needed and always encouraged to ask questions and voice concerns. An initial resident assessment is done, and RN Jovi completes a 14-day, as well as a quarterly assessment and care plan, which the staff can access on the residents’ medical record. The family member responsible, often the Power of Attorney, is then called to review and asked to sign both documents.
Respite and Pet Policies
Unfortunately, we do not have a respite daily rate; however, our residency agreement requires only a 30-day notice to void the agreement. We have a one-time entry fee of $2000. We are a pet-friendly community, requiring a pet deposit of $250 and up-to-date vaccinations. Residents must be able to care for their pets themselves, utilizing our enclosed courtyard if needed.
Availability
We do not currently have a waitlist, although we have varied apartment floor plans and not all sizes are currently available.
Vaccinations
Unfortunately, we are not currently offering COVID vaccinations; however, we do offer the flu vaccination annually onsite.
Aging in Place
Yes, residents can age in place. Please see the admission and discharge criteria, as well as a detailed level of care, included in our brochure information. We currently have availability.
