SOMERFIELD AT THE HERITAGE HEALTH CENTER CHARGES
Effective January 1, 2024
ROOM AND BOARD FEES
Daily Room and Board Fees: includes meals, nursing care, routine ancillary medical supplies, activities, cable TV, and local telephone.
- Private Suite: $495.00 per day
- Deluxe Private Suite: $585.00 per day
- Deluxe Private Suite: Somerfield Life Care residents will pay an additional $90.00 per day above their monthly fee for a deluxe private suite.
FOOD AND BEVERAGE SERVICES
Meal costs:
- Somerfield Life Care Residents: Will be charged an additional $781.00 per month for your two additional meals per day.
- Private Pay Residents: Meal costs are included in your Daily Room and Board Fees.
Guest Meals (applies to families and friends of Somerfield residents):
- Breakfast: $10.25
- Lunch: $13.50
- Dinner: $18.75

Overview of Staff and Training
Our staff consists of Certified Nursing Assistants, Nurses (LPNs, RN, BSN), Nurse Practitioners, Physicians Assistants, MDs, a Registered Dietitian certified in gerontological nutrition, Physical Therapists, PT assistants, Occupational Therapists, OT assistants, and Speech Therapists. Beyond the required training for licensure in each discipline, we undergo training in the "Heartfelt Connections" program, a Life Care initiative designed to meet the needs of not only our memory care residents but also all residents benefiting from its philosophy. Additionally, we have a social worker, visiting dentists several times per year, and a podiatrist.Measures for Resident Safety and Well-being
We ensure frequent rounds and keep residents engaged with daily activities to promote day-night hygiene and improved sleep. PT or OT therapy services are available to assess and aid residents experiencing physical decline. For higher fall-risk residents, we assess room environments for potential hazards, use rubber mats to prevent injury, and employ special pads under clothing to protect legs, hips, or arms. For residents at risk of wandering out of the facility, we use the "WanderGuard" system, which locks doors to the outside while allowing safe residents to come and go as they wish.Support Services for Community Transition
Our licensed Social Worker, alongside our activities staff, excels in helping residents get out of their rooms and engage with others, facilitating a smoother transition. One of our nurse practitioners, who holds a doctorate in psychology, guides the use of medications during the transition period if needed. Each resident's transition experience is unique, with some taking months and others adjusting seamlessly.Transportation Services
We partner with a third-party company, "Caliber Patient Transport," to arrange transportation services as necessary, billed monthly if used.Dining Experience
We are renowned for our food, offering residents a selection of about 7 items for breakfast and dinner daily, including specials and always available items like sandwiches and burgers. We have a full nutritional staff handling in-room meals, with orders taken and served at the table. Our dining rooms are split into two sections: Independent and Assisted. The menu rotates every 12 weeks, and we provide snacks between meals. Our dietitian monitors residents’ labs, weights, and wounds, ordering nutritional supplements such as Boost or Ensure if needed.Resident Complaint Resolution
Complaints or issues are handled by our Social Worker, who collaborates with staff and families to resolve them. We aim to correct any shortcomings and provide contact information for state and local agencies upon admission, displayed at our door.Family Communication
We maintain open communication through a Patient Portal and provide contact information for nurses. We hold formal care plan meetings with the team and family a week after admission and offer them quarterly or as needed.Short-term or Respite Stays
We offer short-term or respite stays at the same daily rate as long-term care: $495.00/day, with a 1-week minimum charge of $3465.00. The respite stay requires a financial application but not the 60 deposit; the length of stay must be paid upon admission.Pet Policy
Pets can visit with proof of current vaccinations, but they are not allowed to reside in the facility.Current Waitlist
There is no waitlist for Long Term Care in the SNF. The ALF has a waitlist, usually prioritizing residents from our Independent Living community as part of our continuum of care contract.Vaccination Availability
We provide Flu, Covid, Pneumonia, and RSV vaccinations within the community.Aging in Place
