Level of Care

Someren Glen has adopted a level of care system that simplifies how assisted services are charged, making it easier to understand and ensuring each person only pays for the level of assistance needed. Assessments to determine the level needed are conducted prior to move-in, 30 days after move-in, upon change of condition, annually and upon request.

Overview of Professionals and Training

At Someren Glen, our staff includes a state-licensed Director, a Director of Health and Wellness (LPN), additional LPN coverage, a lead CNA, a team of QMAPs, and personal care providers. Our life enrichment coordinator and housekeeper are dedicated to our Assisted Living neighborhood. Monthly training is mandatory for all team members, with additional training for care team members.

Our Primary Care Practice includes a Medical Concierge available five days a week and a Nurse Practitioner twice a week for enrolled residents. We also coordinate regular visits from various medical specialists for resident convenience.

Safety and Well-being Measures

Someren Glen ensures resident safety with a 24/7 concierge, personal pendants for AL residents, and a care team member on each floor at all times. We have protocols for regular resident check-ins, care conferences, and courtyard checks. Our focus on relationships helps anticipate and meet residents' care needs.

Transition Support Services

We assist new residents with their transition into Assisted Living through care plan reviews, introductions to care team members, resident ambassadors, and dining escorts. We also provide resources such as moving companies and home care organizations.

Transportation Services

Our community offers scheduled transportation services for appointments, shopping, and other errands from Monday to Friday.

Dining Experience

We have a shared dining room for Independent and Assisted Living residents, with a chef team preparing fresh, hot meals three times a day, plus snacks and beverages for AL residents.

Addressing Complaints and Issues

Residents can provide feedback through multiple channels, including direct access to our Executive Director, comment cards, an external Ombudsman, and Pinnacle Quality Insight.

Cost of Living

The cost of living in our community is detailed in the attached rate sheet for AL, with additional charges for services like garage parking and lost item replacements. A one-time community fee of $2,500 is also applicable.

Communication with Residents' Families

We conduct regular care conferences, use a Voicefriend system for updates, and have an emergency contact system in place to keep families informed about their loved ones' well-being.

Short-term or Respite Stays

We offer short-term respite stays in AL and LTC when available, with costs equivalent to regular move-ins for a minimum of 30 days and a daily rate of $435 for LTC stays.

Pet Policy

We are a pet-friendly community, accepting pets with a one-time fee of $750 in Residential and Assisted Living, provided the resident can care for the pet. A pet agreement and contingency plan are required.

Waitlist Information

We currently have a waitlist for assisted living, which is approximately 90 days.
