Types of Professionals on Our Staff and Their Training
All our staff members are dementia certified to provide the highest quality, skilled care, undergoing four hours of state-specific training and 21 hours of in-depth training. Our nurses additionally complete 16 hours of directly supervised hands-on care with a trainer.

We provide our residents a safe and secure environment, allowing them the freedom to enjoy the outdoors in our lovely community.
Resources and Support Services for Residents' Transition
We offer a monthly dementia support group organized by our Director of Resident & Family Services, Liz Acevedo. Both residents and family members are welcome to attend to share stories and provide support.
Transportation Services for Residents
Our shuttles are available for medical appointments at $5.00 per mile. Additionally, our shuttles are available at no charge for community outings.
Dining Experience
We offer a restaurant dining, fine dining experience featuring nutritious and healthy meal selections customized to accommodate individual dietary requirements and special needs. We also offer complimentary dining for families and friends.
Process for Addressing Resident Complaints
We encourage open communication for any concerns. Residents can communicate their concerns to any of our professional staff members or directly to me. We strive for perfection and understand that addressing concerns promptly is crucial.
Cost of Living in Our Community
Pricing varies based on the level of care your loved one may need. The lowest rate is $350 per day. To provide a more accurate rate, we would perform an assessment to determine the care your loved one will require.
Communication Policy with Residents' Families
Our Director of Resident & Family Services maintains open communication regarding your loved one's care. Any changes in cognition or medication will be communicated to both you and the nursing staff.
Updating Families About Residents' Well-being
We recommend maintaining open communication with our Director of Resident and Family Services as well as our Assistant of Health Services.
Short-term or Respite Stays
Yes, we offer short-term or respite stays at a rate of $410.00 per day with a maximum duration of 30 days.

About Silverado Sierra Vista
Silverado Sierra Vista is a specialty memory care community, taking care of people with dementia from the time of diagnosis until end of life. We have separate buildings/neighborhoods for our residents depending on the level of care they need and the stage of memory impairment they're in.
Early-Stage Neighborhood: Nexus Living
We have a stand-alone early-stage neighborhood where residents enjoy Nexus Living, a specially designed program for those in the early stage of dementia. This program has been clinically proven to slow down the disease process with non-drug interventions such as physical exercise, stress reduction, social clubs, cognitive strategies, and support groups.
Mid to Late-Stage Neighborhood
We also have a neighborhood for those who are in the mid to late stage dementia, ensuring their needs are anticipated by the staff and met.

Our goal at Silverado is to provide a sense of purpose and peace in a warm and nurturing environment for people with memory impairment. The team here, who are all dementia care certified, are tenured and, for us, being at Silverado is not just a job but a vocation.

It also helps that the community sits on three acres of beautiful outside grounds that's fully secure and accessible to our residents, with breath-taking views of the San Gabriel Mountains.

Location
Our community is easy to find - we're around 2 miles north of the 210 freeway when you exit on Azusa Avenue. Azusa Avenue becomes a one-way street as you drive towards the mountains, and we are on the northeast corner of Azusa Avenue and W. Sierra Madre Avenue.
You can drive straight to the parking lot and on the left will be the main entrance to the community. We have a secure community so there's a doorbell and our front reception will be ready to welcome you anytime.
