Community Details
This detailed information was received through an email from the Community Relationship Manager at Sherbrooke Village via email on 2023-12-14.
Overview of Professional Staff and Training
Sherbrooke Village is proud of its highly trained staff! On top of each department having its own uniquely qualified Director, we also have several positions uncommon to most assisted living communities. We have a dietitian, wound care, CNA/LPN/ADON/CMT/SLP.
Safety and Well-being Measures
At Sherbrooke we are pleased to have a 1:5 staff to resident ratio. We also implement secured entrances, wanderguards as well as resident gathering areas for increased supervision.
Transition Support Services
Sherbrooke Village employs 2 Social Workers, each with a focus area in our community, that aid in the transition of residents into our community as well as transitioning to the next level of care. Our Life Enrichment Director has each Memory Care family fill out a multi page "Get to Know You" questionnaire, so that we can be as familiar with a resident's preferences, social history, family as well as triggers and dislikes. Our Community Relationship Manager also focuses a great deal of time getting to know each resident and their family during the tour and admission process. These extra steps ensure that both residents and their families feel at home, and understood as an individual.
Transportation Services
Sherbrooke does provide each resident 2 rides per month at no cost, in our Community Van. We are able to transport to and from medical appointments and even accommodate 3x weekly dialysis appointments. Once those 2 free trips have been used, each resident is offered a variety of transportation services, such as 360 or Medicaid funded transportation.
Dining Experience
Each meal is served to the resident in a dining room, surrounded by their fellow residents, to ensure a socially enriching experience. Meals are all prepared in house. Every day boasts 2 special offerings for lunch and dinner, on top of offering a multitude of alternate choices to make a fully rounded dietary experience. Our kitchen is known for its hand-fried chicken on Thursdays, and home made soups served at each lunch. We are able to accommodate residents on low sodium meals and also have our Dietician on staff to ensure any allergies are met with appropriately prepared offerings. We can assist residents needing mechanical soft diets.
Complaint and Grievance Process
Sherbrooke Village has a Complaint/Grievance process that each resident and their families are educated on during the admission process. We want residents to communicate all issues with us, and have a system in place that each staff member is educated on and expected to follow. Complaints and Grievances are communicated to staff, and then from there determined which Director will be assigned to manage the solution to ensure resident satisfaction. We have highly viable forms located at each reception desk, and modes for anonymous submissions as well as face to face transcriptions.
Cost of Living
We are priced fairly, in reference to our competitors and location. We offer living arrangements starting at studios for $5270/month ranging all the way up to a spacious 2 bed 2 bath units for $6720/month. We are an all inclusive community, and do not charge our residents based on their Level of Care.
Communication with Resident's Families
Residents, within 21 days of admission, will have a care team meeting. Once past the initial care team meeting, residents and their families receive a follow up meeting every 90 days. Nurses also communicate any significant changes that may occur before the 90 days.
Short-term or Respite Stays
Our community can offer respite stays in all levels of care. The cost of the respite is in accordance with the monthly rent charge for each spot. For example, a 30 day respite in our assisted living studio would be the same price one would pay to be a resident for a month. We are able to book a shorter than 30 day respite stay, and in that case divide the month out for a cost per day fee.
Pet-Friendly Community
Sherbrooke is pet friendly! We invite Assisted living residents to bring their pets, and at no extra cost, as long as they can show proof of vaccinations and that they can adequately care for the animal. In other portions of our community, family members can bring their pets in for visits, with the same expectations. We regularly have Duo Dogs, a therapy dog company, come in to offer Pet Therapy to our residents as well.
Waitlist Information
Our community has pockets with waiting lists, mostly for alternately sized living spaces that are currently full, but they are not extensive. We have current availability in each level of care!
Covid and Flu Vaccinations
Our community currently does not currently offer Covid vaccinations. Resident's families would be responsible for making arrangements should they want the most up to date vaccine.
Aging in Place
Sherbrooke does allow residents to age in place. Since we offer the full continuum of care, residents can transition smoothly between all levels, with minimal disruption.
