Our Team and Services
We have a full team of licensed clinical staff. Since we are a skilled nursing facility offering rehab, we have an in-house and fully dedicated therapy department at Shaw. We do not use outside agencies to staff the therapy gym, which we consider a bragging right. 😊 The licensed clinical titles present to staff our building in compliance with State regulation are as follows: MD, RN, LPN, CNA, PT, OT, ST, and RD. Additionally, we have an activities department to ensure a variety of life enrichment activities every single day. We also have a full housekeeping and maintenance department.
Ongoing Trainings
Ongoing trainings are required for all staff at Shaw. Every year, there are several training modules that must be completed and updated. Trainings range from infection control protocols to safe transfer trainings, best practices in the memory care setting (since we have a memory care unit here caring for many dementia residents), medication safety, and many others. Every department has unique trainings and refreshers they must complete every year to stay current and continue employment within the Cascadia network.
Unique Care Plans
Care plans are unique to each resident who admits to Shaw, so they all look a little different. We must follow all MD orders with precision to remain in compliance with state regulations. I feel like I could get exceedingly granular with this answer, because there are many individualized care plan examples of how we support or mitigate certain issues. This piece probably deserves a conversation so I can address your questions with specific answers relevant to what you’d like to know more about.
Social Services
We have a social services department tasked with ensuring safe transitions from Shaw back out into the community. We work with agencies to help find assisted living placement if the patient is not safe to go back home. We coordinate continuity of care with home health, hospice, and home care agencies that will take over once the patient leaves here. We can also help with resources for Medicaid applications if patients need placement but are not financially capable of that without government assistance.
Transportation
Transportation is something we take responsibility for while caring for patients in our building. This includes initial transport to Shaw from wherever they are coming from (unless it is a great distance, like more than 50 miles) and all medical appointments they will have while admitted in our building.
Menu and Dietary Accommodations
We have a menu that comes out every week and rotates so there is always variety. We offer substitutions for items that residents don’t like or may be allergic to. We can accommodate vegetarian diets, cardiac diets, diabetic diets, and any texture or consistency, depending on the medical need of the resident. We serve three meals a day, and residents are encouraged to eat in the dining rooms. We also offer 1:1 assistance for those who cannot feed themselves.
Complaints and Grievances
Complaints or grievances are filed with social services, either by the resident or resident family, and we take time every morning in our department head meeting to address any that have been filed. Resident complaints and concerns are handled and resolved promptly with our entire leadership team present. We keep a strong finger on the pulse of areas in our building which need attention.
Room Rates
A private room is $340 per day, and a shared room is $298 per day. We do take Idaho Medicaid and most long-term care insurance policies.
Communication with Families
Cadence of communication back to families and general updates are set up by social services. Upon admission, we like to schedule a care conference so that everyone can participate and discuss the specifics of how a resident/family likes to be communicated with. Any communications that need to happen with specific frequency are incorporated into the resident’s care plan.
Respite Services
We offer respite services but require a 7-day minimum length of stay. We still must have a completed MD order to admit, even if it is just respite.
Pet-Friendly Policy
We are definitely pet-friendly and encourage them to visit residents as often as you’d like. They cannot stay overnight, though.
Vaccinations
We offer COVID vaccines, as well as all other routine vaccinations. They can be done here at Shaw or in the community.
Long-Term Care
Since Shaw is a skilled nursing facility, we always plan to keep residents indefinitely when they move in for long-term care. We are able to care for all stages of life and most any disease process, up until the very end. Currently, we do not have any long-term care beds available, except for one opening for a female resident in our memory care unit called the Friendship House. If this may pertain to what you are looking for, I’d love to chat about that! As for future availability: I used to keep a waiting list, but the list grew longer than I could manage, and by the time we had open beds, family members had found other arrangements. So, it ended up being low-yield for the amount of work it took to manage.
