Do you offer transportation services for residents?
We do offer transportation for our residents. We also have a van request sheet that we provide to the residents. Please see the attachment above for van requests.
Could you tell me a little about your dining experience?
We offer a restaurant-style dining experience for our residents. We provide 3 meals a day (breakfast, lunch, dinner) of which each meal has 2 different entrée options for our residents to choose from. We also have an a la carte menu available.
Do you all offer short-term or respite stays? How much does this cost?
We do offer respite care. Respite stays are a daily rate of $300 and $350 for our memory care. Please see the attachment above for our Respite rates.
Are you all a pet-friendly community?
We are pet friendly. We have a one-time pet fee of $500.
Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?
We employ med techs and resident aides to provide care to our residents. They are trained as “Direct Care Staff” per DHS regulations. Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
All of our residents are provided with pendant “call bells” and there are pull cords in each resident bathroom. We also develop an individualized Residents Assessment and Support Plan to address the specific needs of each individual. Safety measure can look different for each person, and can include mobility devices, staff check-ins, etc.
Do you offer any resources or support services to aid residents' transition into the community?
We do not have any formal support services in place for this. However, I do think we do a good job of welcoming residents, getting to know them, and making them feel comfortable in our environment. We work closely with the family to make sure that the expectations and care needs of the residents are being met from the start. The first few weeks are definitely an adjustment period.
What is the process that takes place if a resident has a complaint or an issue they want addressed?
Our management and lead team are available to field concerns. The majority of the time, there is a lead or manager in the building, but in the case there is not, there is a person on call that can field and address the concern.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
We have one family member designated as the emergency contact in the resident’s medical record. This is the family member that is notified in the case of an emergency or significant change in condition. Our front desk is staffed 24/7, so if a family member would like to call in and check on their loved one, there is always someone to answer the phone and take the call.
Are residents able to get Covid vaccination and flu vaccinations in the community?
Yes, we have pre-scheduled covid and flu vaccine clinics. We schedule them seasonally or as new boosters become available.
Are residents able to age in place?
Yes. We are able to bring in hospice as the resident declines to assist in managing the resident’s care. Our care thresholds are as follows: we do not utilize any type of mechanical lifts. We cannot accommodate residents that require a consistent assistance of 2 staff to transfer. If this occurs, we would be looking to assist the resident and family to make a transition to a nursing home. We do not have a nurse on staff 24/7, therefore we cannot accommodate injections (other than insulin or epi pens), tube feedings, IV’s, and any high-level clinical needs that require licensed nursing monitoring.
We do have immediate openings both in our general Personal Care home and on our secure dementia unit.
