Our Professional Team
Our facility has the following professionals: Certified Nursing Assistant, Wound care nursing specialist, Nurse practitioners, Psychiatric nurse practitioners, Eye physicians, Podiatrist, RNs, LPNs, Medical Director and we train certified nursing assistants on-site. We meet the federal guidelines for the required yearly training hours. New products/procedures are in-serviced on an as-needed basis.
Committees and Education
We have ongoing safety committee and fall committee with educational classes. We are a restraint free facility.
Transition to Home
Our therapy and social services departments work to provide a safe transition to home. They work with many vendors for Home Health, equipment, companion services.
Transportation Services
Transportation services are provided for medical appointments for those whose insurance does not cover that services. Special recreational events are also provided some transportation by the facility.
Dining Experience
Dining can be enjoyed either in-room or the dining room. Feeding assistance is provided at no extra charge. Snacks are available throughout the day as well as through some of our Life Enrichment programs.
Social Services
Our social service department is tasked with addressing all resident/family concerns. All issues are investigated and addressed.
Room and Board Rates
Room and board rates are as follows: Rehab Suite - $460 daily; Private room - $300 daily; Semi-private room $285 daily. Medical supplies and medications are charged for on an 'as issued' basis. Our pharmacy provider is Omnicare.
Communication with Family
Family are given a choice on contact options. The most commonly used is phone communication. Next are email followed by text messaging.
Respite Care
Respite care is offered on an available bed basis. Rates are the same as listed above per day. Must meet the state guidelines of a History/Physical to qualify for a respite bed. If a contracted hospice agency, they may have respite stay coverage for which they pay.
Pet Policy
Pets are welcome here. They must have proof of all state/local required vaccines, not be present in the dining/activity area when food is being served, leashed while in the building.
