Staff Overview and Training

• In-House PCP: Dr. Patel and nurse practitioner available Tuesdays and Thursdays
• Nurses: 4 Registered Nurses (RNs)
• Aides: Resident Assistants (RAs) and State Tested Nursing Assistants (STNAs)
• Training: Dementia Training, First-Aid, Fire Safety, Behaviors, Resident Rights, HIPAA, Hoyer Lift
• 360 Psych (optional service)
• Podiatry (optional service)

Resident Safety and Well-being Measures

We can put the resident on well-being checks in the middle of the night to ensure safety when going to the bathroom. At every meal, we take attendance and if not present we do a well-being check. When the nurses distribute the medications that is another well-being check. We do have a clinic at the beginning of every month to check all the residents' blood pressure, temperature, and weight.

Transition and Support Services

We have a transitional service for $150 for 3 days involving assistance with ADL’s (A.M./P.M.), one bath and escort to/from meals and activities for three days after move-in. We do have daily activities, which includes bingo, exercise, crafts, movie days, and outings to grocery stores and restaurants.

Transportation Services

We do not have a bus driver, resulting in getting transport through insurance for outside appointments.

Dining Experience

Our dining room offers three meals a day to our residents. It is restaurant style, with servers bringing beverages and meals. There is an assigned meal for the day, but alternative options are available.

Addressing Resident Complaints

If a resident has a complaint or issue, we suggest seeing the management team or addressing clinical concerns to the nurse on staff or Director of Nursing (DON) if present.

Cost of Living

The cost of living is detailed in the previous email attachment. The room and board rate for a studio is $2,940 and one bedroom is $3,360, including meals, housekeeping, laundry, and stand-by bathing assistance. Additional care incurs extra costs. We accept the AL Waiver through the Western Reserve Area on Aging.

Communication with Families

The policy on communication with residents' families involves updates from nurses if there is a change in status. Families can call anytime to inquire about their loved ones.

Short-term or Respite Stays

We offer respite stays at $150 per day.

Pet Policy

We do not accept pets in our facility.

Availability and Waitlist

We do not have a waitlist and currently have one-bedroom and studio apartments available.

Vaccination Clinics

We just had a flu vaccination clinic and are planning a COVID booster clinic in the coming weeks.

Aging in Place

Residents can age in place. If a resident requires hospice care, they can stay at our facility. Otherwise, we may need to transition them to a Long-Term Care facility if they require more intensive care.
