Semi-private room: $194.00/day

Private room: $224.00/day

Overview of Staff

I have registered nurses, licensed nurses, and certified nursing assistants. We have physical therapists that will assist with mobility, occupational therapists that will assist with activities of daily living, and speech therapists that will focus on any cognitive issues or swallowing issues to provide strategies.

Safety Measures

In place, we provide 24/7 care, rounding on residents, and if needed, other safety measures such as floor mats. We will tailor the need on an individual basis.

Complaints or Issues

Are addressed immediately via a nurse manager, social worker, or myself, the administrator.

Transition into Community

We provide therapy and a social worker to assist with anything you may need post-discharge.

Cost of Living

Our facility for long-term care is as stated above. Ancillary services will be discussed further based on needs. We also take Medicare and Medicaid.

Communication with Families

We have care plan meetings monthly, but we encourage family communication as often as needed and requested by you. I keep an open door policy, as well as my social worker, nursing, and activities department.

Dining Experience

We have three square meals that are dietician approved based on their medical recommendations from a physician.

Complaints are addressed via our grievance process. We will investigate immediately and come up with a solution and or recommendations.

Respite Care

Is provided at our facility, pay rates are as indicated above for room and board, based on individual needs.

Pet Friendly

For visits is ok if shot records are available to us and the dog is well trained.

Immunizations

Are offered in our community and residents are able to age in place.
