Waitlist and Availability

No waitlist here at our Weston campus as we typically don’t have a lot of openings on our Long Term unit. First come first serve on our beds here.

Pricing

Private Room is $425.00 per day, semi-private is $375.00 per day.

Staff and Training

We have Registered Nurses, LPN’s, and CNA’s on staff here as well as Nurse Practitioners in house 5 days per week through our off-site Medical Director agency. I would need more clarification on what you mean by training.

Safety and Well-being Measures

All of our residents here are asked to use their call light to ask for assistance. We do have a wanderguard system in place on our doors. We are not a locked unit (like a memory care facility would be).

Transition Support Services

We have 2 Social Workers on staff to help coordinate any care needed after short-term rehab is complete.

Transportation Services

Admission ride is free of charge, appointment transportation is resident responsibility ($50 round trip plus $2 per mile).

Dining Experience

Each resident has the opportunity to choose what they’d like to eat each meal based off our daily menus. Typically there is two main entrée options to pick from, and if neither of those are preferred there is an Al A Carte menu to pick from. Residents have the right to eat in their room or the dining room, but we do encourage dining room experience as it presents a wonderful atmosphere during meal times.

Handling Complaints and Issues

Resident would report their complaint to any staff member, our IDT team becomes involved to address the complaint to offer resolution.

Communication with Residents' Families

Resident would have to grant permission for us to release any type of information to begin with, then would have to specify who is able to receive that information. If your name isn’t on the approved list by the resident, we are not able to release information and would direct you to speak with the resident or their immediate family members if applicable.

Keeping Families Updated

If Resident approves family to receive information, we ask that they assign one of their emergency contacts to also be a “Resident Representative” which means that representative would be responsible to relay any information given to the rest of the family or contacts.

Short-term or Respite Stays

Yes

Could be covered under insurance if the resident has a medical necessity for rehab, otherwise respite would be the same cost listed above.

Pet Policy

For visitation only. Must be up to date on rabies and kept leashed.

Vaccinations

Yes, residents are able to get Covid vaccination and flu vaccinations in the community.

Aging in Place

As long as we are able to continue to meet their needs, yes. No waitlist here at our Weston campus as we typically don’t have a lot of openings on our Long Term unit. First come first serve on our beds here.
