Overview of Staff and Training
Our team comprises highly skilled professionals including an NHA licensed Administrator, RN/LPN Director of Nursing and Resident Care Manager, Social Worker, Activity Director, Dietary Manager, Maintenance/Housekeeping staff, Medical Director, and Registered Dietician (part-time/on-call).
Safety and Well-being Measures
We adhere to the standards outlined in the last two pages of the facility reporting incident form. Our exits are equipped with numerical codes to enhance security.
Resources for Transition into the Community
We provide comprehensive discharge planning support through our full-time Social Worker.
Transportation Services
Transportation to medically relevant appointments is covered for residents using their Medicare Part A benefit for a skilled stay. Medicaid also covers private medical transport. For private-pay individuals, options include Dial-A-Ride or out-of-pocket expenses. We have a bus available for shopping trips and non-medical appointments, and staff can assist in scheduling transport.
Dining Experience
We offer a 5-week rotating menu that changes twice a year. Basic alternatives are always available. Residents can choose to dine in our dining room or in their own rooms.
Handling Complaints or Issues
Grievance forms are available, and a designated Grievance Officer ensures timely resolutions to complaints. The licensing complaint number and Ombudsman’s number are posted throughout the building.
Communication with Families
Initial care conferences are held 7-14 days after admission and thereafter quarterly. Nurses are available 24/7 for care-related inquiries. Families are notified of any significant changes such as medication adjustments, falls, bruises, pressure sores, or scratches.
Short-Term or Respite Stays
We offer short-term or respite stays pending availability. The basic daily rate is $550/day. Hospice Medicare/Medicaid and most commercial insurances cover the full cost of a 5-day respite stay.
Pet Policy
Pets are welcome to visit, provided we have updated records for them. However, residents are not allowed to have pets live with them permanently.
Vaccinations
Seasonal flu and COVID-19 vaccinations are available to residents upon request according to CDC recommendations.
Aging in Place
We offer the highest level of long-term care, suitable for residents who are completely dependent on others for all ADLs. Currently, we do not have openings but we maintain a waiting list and review potential admissions as vacancies arise.
