Additional Rate Information

• Sunflower Wellness Program: $5,495 per month + Care Level
• Hospice: $500 each day for first 2 days, $295 for additional days, Minimum 2 day stay
• Respite Care Rates (2 week minimum); $275 per day based on Care level and room selection
• Pre—Admission Community Fee: Long Term $3000/ Short Term or Hospice $750

Overview of Professionals and Training

We have a full staff of LVNs, a Doctor on staff, and trained Care Givers with a ratio of 1:6. Our Regional Director is a Gerontologist and our Sunflower Program Director is a Case Manager/Social Services professional. We also have an in-house Pharmacy.

Safety and Well-being Measures

The facility is not a closed site except during the hours of 8:00pm – 8:00am. Surveillance Cameras and Sensors are at all doorways. Call buttons in each room alert the Med Tech Room and Nurses’ Station. We have 16hr/7 Nursing Staff and 24/7 Care Givers.

Resources and Support Services

We work with three Third Party programs for low-income prospects: Assisted Living Waiver (ALW) through the Institute on Aging, Master Care Plan, and On Lok. We assess all prospects before admission to understand their medical, physical, and mental health needs.

Transportation Services

We do not have our own transportation but schedule transportation for hospital and doctors’ appointments. Some residents use SF Paratransit.

Dining Experience

Our chef creates a variety of dishes with different cuisines, offering two choices for lunch and dinner. We address all dietary restrictions and religious observations. Meals are served by staff, and we encourage dining room meals, mobility permitting. Room service is available for those who wish or are not feeling well. We celebrate birthdays, holidays, and special events.

Addressing Resident Complaints

The resident contacts the medical staff and Executive Director to address any complaints or issues.

Cost of Living

Cost depends on the floor (starting at $3,895) and Level of Care (starting at $500). We do an assessment to determine the appropriate floor and Care Level. The one-time Community fee is $3000, with a month-to-month contract. The monthly fee includes everything except for cell phone and certain transportation costs.

Communication with Residents' Families

When an incident or hospitalization occurs, the family and/or Responsible Person is notified. Family members can call or email me anytime.

Short-term or Respite Stays

Respite stays have a $500 Community Fee, $275.00 for a shared room, and $300.00 for a private room, with a 2-week minimum stay.

Pet-Friendly Community

We do not have pets living with residents, but they are welcome to visit. Currently, we do not have a waitlist.
