Resident can perform Activities of Daily living, dressing, grooming, toileting, bathing, eating, bed mobility and ambulation with staff reminders, set up and cuing but does not require hands on assistance from staff and is able to manage toileting independently. Does not require any assistance with eating. Includes 2 Showers per week, two loads of laundry, (1clothing, 1 linen), and weekly housekeeping.

Resident requires minimal assistance from staff to perform one or all Activities of Daily Living, dressing, grooming, toileting, bathing, eating, bed mobility and ambulation. Minimal assist indicates assistance with shoes, socks buttons, SBA for transfers, ambulation, and bathing. Resident can participate in dressing, grooming, and bathing and is able to manage their own incontinent products with set up. Resident may require food to be set up or cut up. Includes 2 Showers per week, two loads of laundry, (1clothing, 1 linen), and weekly housekeeping.

Resident requires a moderate amount of assistance to perform one or all Activities of Daily Living, dressing, toileting grooming, bathing, bed mobility and ambulation. Moderate assist indicates the need for 1 staff person to physically assist resident, while resident can still participate staff performs 50% of the physical tasks of any of or all the following dressing, grooming, bathing toileting, transferring, bed mobility and ambulation, resident requires hands on assist with toileting and managing incontinent products. Resident may require help cutting food, reminders and cuing to finish a meal. Includes 2 Showers per week, two loads of laundry, (1clothing, 1 linen), and weekly housekeeping.

EZ stand or Hoyer lift for mobility. Resident may require some or total assistance to eat. Includes resident at the stage of “end of life” Includes 2 Showers per week, two loads of laundry, (1clothing, 1 linen), and weekly housekeeping.

EZ stand or Hoyer lift for mobility. Resident may have disruptive behaviors or behaviors that require re-direction or intervention. Resident may require some or total assistance to eat.
